Update Provider Name Information

The NPI number associated with the selected provider is found in the NPI registry, but the demographic information in the registry does not match the information present in the Surescripts provider record.

Specifically, one or more of the following conditions apply:
  • Last name does not match the registry's Last Name or Other Last Name values
  • First name does not match the registry's First Name or Other First Name values
  • Middle name does not match the registry's Middle Name or Other Middle Name values
  • Gender does not match the registry's Gender value
  1. In File Maintenance, open the provider in the Providers master file.
    The Modify Provider Information window opens.
  2. Select the Demographics tab.

  3. Update the provider's name to match the legal name the selected provider has on file with the National Provider Registry.
    For example, provider name displays as Jon Smith, however the National Provider Registry includes Jonathan Smith in its record. In this example, you would need to change first name to Jonathan for the selected provider.
  4. Select the ePrescribing tab.
  5. In the Networks section, select open menu .
  6. Select Retail > Enroll.
    The EPrescribing Message Submission window appears.
  7. Select OK.
