Update Provider Name Information
The NPI number associated with the selected provider is found in the NPI registry, but the demographic information in the registry does not match the information present in the Surescripts provider record.
Specifically, one or more of the following conditions apply:
- Last name does not match the registry's Last Name or Other Last Name values
- First name does not match the registry's First Name or Other First Name values
- Middle name does not match the registry's Middle Name or Other Middle Name values
- Gender does not match the registry's Gender value
