Open the Problem Notes

Add notes to a problem in Problems module in the Problem Notes window. The View/Add Notes button enables you to view or add notes to a problem in the Problems module. Select the patient's encounter that contains the problem. Select the problem in the Problem List. Do one of the following: Right-click the problem, and then select View/Add Notes. On the problem list data entry area toolbar, select View/Add Notes. The Problem Notes window opens. Problem Notes Set Column Preferences for Problem Notes Window Set the display preferences of columns in the Problem Notes window. Add a Problem Note Add relevant information about a problem as a note in the patient chart. Update a Problem Note Edit the problem note or add more information in the Problem Notes window. Problem List Data Entry Area Toolbar

The problem list data entry area toolbar is located at the top of the data entry area and below the data display area. The data entry area toolbar buttons enable you to perform functions related to a patient's problems such as add, recode, and modify the problems. Re-Code a Patient Problem in the Problem List Resolve an Active Problem Set a selected active problem to the resolved state and provide details such as the Resolved Date, Resolved By, and Resolved Reason. Mark a Problem as Chronic Mark recurring or long-standing problems as chronic. Mark a Problem as Comorbid Mark co-occuring problems as comorbid. Educational and Referential Resources Access educational and referential resources from the problem list data entry area toolbar of the Problem List tab or the Billing ICD List tab. Add Problem to the Problem List

To open the Problems window, do one of the following: In the Problem List panel, click Add/Update Click Problems in the patient information bar. The Problems window opens. Enter a problem in the diagnosis search window and click Search. Highlight the problem from the list and click Select. The system fills the associated values in the Fully Specified SNOMED® Description, Problem Description, ICD Description and ICD Code fields in the Problem List panel. For all new problems, Mark as Chronic is selected by default. Enter the Clinical Status. Enter the Onset Date. Enter the Resolved Date. If the problem is not chronic, do one of the following: Select Secondary Condition to mark the problem as a secondary condition. Select Marked as Tracked Problem to add the selected problem to the tracked problem list. To add the problem to the assessment list, select Add to Today’s Assessments. Add a New Problem to a Patient's Problem List

Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. On the Care Transitions tab, expand the Problem List after Discharge panel, and then click Add. The Problems window opens. Click Add Problem. A search window appears. Add Problem Window Note: Problems are stored in the database and displayed using SNOMED® CT (SNOMED Clinical Terms). Search for the problem by typing a full or partial name of the problem in the search box. Note: The Add to My Tracked Problems option is selected as a default. Click the desired problem. Click Select to display and add any additional details. Make any necessary changes to the problem. Click Accept to add the problem to the list. Problem List

The Problem List tab enables you to view the details of the active, inactive, resolved, and unresolved problems of a patient and update them as needed. You can record and update a patient’s problems using the Systemized Nomenclature of Medicine - Clinical Terms (SNOMED-CT®) concept. SNOMED-CT® is a standard vocabulary used to express clinical terms. SNOMED-CT® is comprised of descriptive terms or synonyms related to a clinical concept and provides links between different concepts. The problem list is demographic and is not associated with an encounter. After you record a problem if you need to use it as a diagnosis for billing, you must add the problem to the billing ICD list. Add a New SNOMED Code Mapping

You can add a SNOMED mapping in the SNOMED Configuration practice template. Access the SNOMED Configuration practice template. Right-click the SNOMED grid and select Add New. The SNOMED Mapping - [New Record] template opens. Enter a field name, and a symptom in the appropriate fields. Enter a concept in the Concept ID 1, Concept ID 2, and Concept ID 3 fields. Enter a SNOMED code in the SNOMED Code 1 and SNOMED Code 2 fields. Click Saveand click Close. The information entered appears in the SNOMED grid.
