You can change an existing SNOMED® concept ID to another SNOMED® concept ID by changing the concept ID, description, and fully specified name while maintaining all other details of the original problem.
Select the patient's encounter that contains the problem to re-code.
Select a problem in the Problem List.
Do one of the following:
Right-click the problem, and then select Re-Code.
On the problem list data entry area toolbar, select Re-Code.
The Problem Search window opens.
Enter the partial or total description of the problem, and then select Search.
The search results appear.
Select the applicable problem from the search result list, and then selecting Select.
The Problem Search window closes, and the re-coded problem replaces the original problem in the patient's encounter.
