Medical Necessity Setup

You must set up the NextGen® Medical Necessity Check to determine if Medicare will pay for a test when selecting a diagnosis for an order and run it when you place a test order. If Medicare or other insurance does not cover the costs of the test, you can print an Advanced Beneficiary Notice (ABN) and record the patients' decision about how to handle test codes. If an order does not contain Medicare or other insurance then you can't run NextGen® Medical Necessity Check as this follows the Medicare guidelines.

  1. To set up the NextGen® Medical Necessity Check locations, do the following:
    • Access and download the NextGen® Medical Necessity Check update package from the NextGen Healthcare Success Community. This package populates the Locations (and associated data) in the Orders Module tab.
    • Set up the appropriate NextGen® Medical Necessity Check encounter level insurance for the appropriate location.

  2. Assign the appropriate CPT4 codes to the order tests.
  3. To indicate that NextGen® Medical Necessity Check should be run automatically when an order is saved, enable the practice preference Run Medical Necessity Check when order is saved.
  4. To indicate if the NextGen® Medical Necessity Check should skip a test if no CPT4 code is assigned, select the Practice Preference Skip Medical Necessity Check if no CPT4 code.
  5. To run NextGen® Medical Necessity Check against insurances other than Medicare, you must set those payers up in File Maintenance.
  6. Set up the Advanced Beneficiary Notice (ABN) report. Ensure that you set up the ABN report (file name: ABN_OM.rpt) when setting up the Orders Crystal Reports. This enables you to access this report when the NextGen® Medical Necessity Check indicates that a test for a patient cannot be covered.
