Create a Report Header and Footer
You may choose to create a specific Header and/or Footer for your report.
Note: By default, each NextGen® Enterprise EHR Report has a Header that displays Practice Title and Date Prepared (formatted as 1/1/xx), as well as a Footer that displays Page Number (formatted Page 1).
- From the Report Filter, select Head/Foot.
The Customize Report Header/Footer window opens.
- Select the options you wish to be displayed in the Header of your report:
- Main Title - Enter the desired Main Title
- Report Title - Enter the desired Report Title
- Sub Title - Enter the desired Sub Title
- Date Prepared - Select the format of the Date Prepared displayed
- Page Number - Select the format of the Page Number displayed
- Practice Title - Select whether to use practice's name
- Practice Logo - Select whether to use your practice's logo
- Print Header on All Pages
For each selected option, choose whether it is displayed on the Left, Center or Right side of the Report.
- Select the options you wish to be displayed in the Footer of your report:
- Free Form Text - Enter the specific text you wish to be displayed in the Footer of the report
- Date Prepared - Select the Format of the Date Prepared displayed
- Page Number - Select the format of the Page Number displayed
- Print Footer on All Pages
For each selected option, choose whether it is displayed on the Left, Center or Right side of the Report.
- If necessary, select and enter Addendum Lines 1-5. For each selected Addendum Line, choose whether it is displayed on the Left, Center or Right side of the Report.
- If necessary, select Report Filter Definition check box to include display of all filters used on the report (grouped by Report Filter Tab) in the Footer of the generated report.
- Select OK.
