For the FTS Finalize or Ophthalmology Finalize template, you must enter the effective date for the 2021 Coding Guidelines in a template you open from the EM Coding section of the Ngkbm Config template or the Practice Configuration template. You can set a date prior to the actual effective date for testing purposes. From the File menu, click System\Practice Template. The Select Template window opens. Click the Practice tab. In the Show section, select All. A list of practice-level templates appears. Do one of the following: For NextGen® Adaptive Content Engine, 8.4 to 8.4.1, click ngkbm Config, and then click OK. The Ngkbm Config template opens. For NextGen® Adaptive Content Engine, 8.4.2 and above, click Practice Configuration and then click OK. The Practice Configuration template opens. Before you begin To submit an E&M code, you must have selected an assessment in the Today’s Assessments panel. For more information on assessments, go to NextGen Healthcare Success Community and download the latest Getting Started User Guide forNextGen®Adaptive Content Engine. Open the Finalize template and expand the Evaluation and Management Coding panel. Evaluation and Management Coding To use a calculated E&M code, review and select the MDM level or enter the total time. For more details, see Calculate an E&M Code under 2021 Coding Guidelines on the FTS Finalize Template. The Medical Decision Making section displays the prepopulated MDM level based on the calculated levels in the three tables in the Medical Decision-Making Elements panel. To bypass or override the calculated code, select an E&M code in the code option section at the bottom of the Evaluation and Management Coding panel. Note: Code 99201 cannot be selected. After you install the update package for NextGen® Adaptive Content Engine, you can perform Evaluation and Management (E&M) coding under 2021 Coding Guidelines for the FTS Finalize, Ophthalmology Finalize, and Cardiology Finalize templates. For information on installing the package, go to NextGen Healthcare Success Community and download the latest 2021 E&M Coding Updates Installation Guide for NextGen® Adaptive Content Engine. The 2021 Coding Guidelines calculate E&M codes based on one of the following: Medical Decision Making (MDM) level selected Total time spent on patient care and documentation for the visit, which includes the face-to-face time with the patient and other time personally spent by the physician and/or other qualified healthcare professionals on the day of the encounter You can include counseling time in the total time, but the 2021 Coding Guidelines do not require that over 50 percent of the total time be spent in counseling or coordinating care. To calculate an E&M code under the 2021 Coding Guidelines, you must select the Medical Decision Making (MDM) level or enter time on the EM 2021 Guidelines template that opens from the Ophthalmology Finalize template. To compare E&M codes, you can switch between the calculation methods. However, you cannot combine codes from both methods. The application uses the last calculated code. Open the Ophthalmology Finalize template and expand the EM code panel. Click Calculate code. The EM 2021 Guidelines template opens. EM 2021 Guidelines template To calculate an E&M code on the MDM level for a new or an established patient, do one of the following: Select a complexity level. For information on complexity levels and risks involved in MDM, click the View MDM Guidelines link. If the complexity level has been carried over from the Ophthalmology Finalize template, select a complexity level again. The calculated code has been selected for the new or the established patient. You can calculate E&M coding and submit charges to billing on the Finalize - OPH template. In Ophthalmology, providers can choose to use the '97 or the 2021 E&M Code calculation or one of the codes specific to Ophthalmology referred to as Eye Codes. Prior to submitting codes to the Superbill, you can link procedures to an appropriate diagnosis, and select whether to bill for an encounter that was procedures only or an encounter where no procedures were performed. You can review E&M coding calculations and override, if appropriate, and bill for counseling services. Effective January 1, 2021, the E&M code is automatically selected based on the 2021 E&M Coding Guidelines, in which Medical Decision Making (MDM) complexity level or the total time spent with patient care and documenting the encounter is used to determine the code level. You can submit a calculated or selected Evaluation and Management (E&M) code to the Superbill using the 2021 Coding Guidelines. Before you begin The date of the encounter must be on or after the effective start date entered for the 2021 Coding Guidelines on the Practice Configuration template. The visit type must be one of the visit types impacted by the 2021 guidelines. For more information on the 2021 Coding Guidelines, go to NextGen Healthcare Success Community, and download the E&M Coding User Guide forNextGen®Adaptive Content Engine. Also, you must select an assessment in the Today’s Assessment panel to be able to submit an Evaluation and Management (E&M) code. For guidance on how to use the 2021 E&M Coding Guidelines based on Medical Decision Making (MDM) level and time, you can use the information Info Icon icon. For guidance on the level of complexity and risks involved in MDM, you can use the View MDM Guidelines link. From an encounter, select the Templates Template Icon icon.
