Patient's Adult Social History

You can view and document the social history of an adult patient. You can document the social history information of the patient in a one-page panel format on the Adult Social History template. This template does not appear by default. Adult Social History The Social History Review section enables you to document the review status. You can select an option to complete the social history review. You can select the Enable Panel version of Adult Social History check box in the Social History section of the Practice Configuration practice template to set the template as default. Confidential Information

You can use the confidential history templates to document a patient's confidential information. You can open the confidential history templates by selecting the following links. The Confidential History link in the Social panel on the Histories template. The Confidential link in the History Summary panel on the Intake template. The links enable you to document the following confidential information of the patient. Alcohol and drug use On the Confidential - Alcohol/Drug template. Psychiatric history On the Confidential - Psychiatric template. Abuse or domestic violence and incarceration On the Confidential - Abuse/Incarceration template. Sexual practices and STI details On the Confidential - Sexual Practices/STIs template. With the patient's permission, you can generate the documented confidential details in the social history document. You need to select the Include all confidential information in the social history document with patient permission check box at the top of the template

Confidential Information

You can use the confidential history templates to document a patient's confidential information. Document Social Confidential History

In the Histories template, scroll to the Social panel, and click the Confidential History link. The Confidential history template opens. Enter confidential history. Click Intake Note. Click the SOAP tab. To generate the Master document, do one of the following: Scroll to the bottom of the SOAP template, click Generate Note and select Master document from the picklist. Patient's Tobacco Use Information

You can document the patient's current or past tobacco use information. You can document the patient's tobacco cessation efforts, vaping use details, and exposure to passive tobacco smoke in the following panels on the Social History - Tobacco template. Tobacco Use Historical Use Efforts To Quit Tobacco Vaping Use Passive Exposure Social History - Tobacco You can open the Social History – Tobacco template through the navigation bar, the History Summary panel on the Intake template, the Social panel on the Histories template, or the Risk Factors Config template. Confidential Social History Configuration

The Show Confidential History check box in the Practice Configuration template enables you to include confidential information in the social history document with patient permission. Social History section in Practice Configuration When the Show Confidential History Information on Master Document check box under Practice Level Configurations in the Document Configuration practice template is selected, the confidential history information gets generated in the master or intake document. Practice Level Configurations in Document Configuration Note: When the Show Confidential History check box is selected on the Practice Configuration practice template, the system automatically selects the Show Confidential History Information on Master Document check box on the Document Configuration practice template.
