Update Practice Care Team
From the Care Team Configuration template, select a team from the Practice Care Team Details grid. The details of care team and its members appear in the corresponding fields. To update the care team details, edit the status and/or active and inactive dates. To update the care team member details, edit the status and/or active and inactive dates, and role or function. Add Practice Care Team
You can create a practice care team and add members to the team from the Practice Team Configuration panel of the Care Team Configuration template. To create a practice care team, you must add at least one member to the care team and there is no restriction on the maximum number of members that you can add. You can add two types of participants to a practice care team: Clinical professional and Agency/Organization. From the Care Team Configuration template, enter the care team name. Care Team Configuration template Select a care team status.By default, the status of the care team is Active. To update the active and inactive date and time of the practice care team, enter the details in the corresponding fields.By default, the active date and time is the current date and time. To add the member details, specify the participant type and do one of the following: Add the member from NPI Registry lookup. Generate Risk Score Review for a Practice Care Team
From the Care Team Configuration template, in the Practice Care Team and Risk Score Review section, do one of the following: To apply the risk filter across all care teams, select the All Teams check box. To apply the risk filter to a single team, select a care team. Practice Care Team and Risk Score Review section Do one of the following: In the Include risk levels, select the risk levels you want to filter by. To include all risk levels, select All Click Apply. Care Team Configuration
You can configure care teams from the Care Team Configuration template, in Practice Team Configuration and Practice Care Team and Risk Score Review panels. To configure a care team, do the following: Create practice care teams Add members to the team and assign primary providers Update and remove members from the practice care team Filter the care team and member details based on the team name and member status View patients and their risk scores associated to teams Generate Practice Care Team and Risk Score Review reports Care Team Configuration template Add Practice Care Team Update Practice Care Team You can update status, active details, inactive details, and member details of a practice care team from the Care Team Configuration template. Filter Practice Care Team and Members
Open the Care Team Configuration template. From the Filter by team name list, select a team. The practice care team and member details appear in the Practice Care Team Details grid. To filter member details based on their status, select a status from the Filter by status list. The member details appear in the Individual Care Team Members Details grid.
