template opens.
Do the following:
Verify the location of the finding to be treated in the
Location
section.
If the location is incorrect or missing, enter the location information or click
Add
to select the location from the image.
Document relevant history, associated symptoms/pertinent negatives, and other relevant patient information.
Click
Save & Close
.
On the
Intake
template, record all other clinical and demographic data as required by your practice.

Access and Document Mohs Surgery Procedure
The provider workflow for Mohs surgery consists of these general steps. On the Intake template, select the Specialty as Dermatology and Visit Type as Mohs Surgery Visit. Click the Procedure tab. The Derm Proc Mohs 1 template opens. In the Pre-Procedure panel, document the required information. In the Mohs panel, document the required information. In the Repair panel, document the required information. In the Post Procedure panel, document the required information. Submit the procedure to the Superbill. Document Mohs Pre-Procedure Details You can document the pre-procedure verification steps and identify the surgeon and assistant. Document Mohs Site Details You can review and update the details of the skin cancer record before Mohs surgery treatment. Enter Mohs Surgery Details You can document detailed information about each stage of the Mohs surgery procedure in the Mohs panel. Finalize the Mohs Visit
On the Finalize - Mohs Procedure template, the provider can verify and submit all charges for the encounter. You should not submit charges until you have completely documented the encounter, ensure that the most accurate codes and units are submitted. On the Intake template, select the Specialty as Dermatology and Visit Type as Mohs Surgery Visit. Click the Finalize tab. The Finalize - Mohs Procedure template opens. In the Procedure panel, if you do not want to have any charges submitted, click the Surgery hold check box. The check box can be cleared later and then charges can be submitted. To add a modifier to a service item, double-click the item to open the Procedure Billing Ext template. Make the necessary changes. If you do not want to have an item included in the billing, click the No Charge check box. When complete, click Save, and then click Close. General Workflow for Mohs Surgery
The general workflow for the Mohs surgery process includes the following tasks. An office encounter during which the lesion is identified as potentially cancerous and a biopsy is ordered. A biopsy encounter during which the biopsy is performed. A chart encounter during which pathology results are documented and added to the Skin Cancer History Log. When recording pathology results for skin cancer, you specify recommended treatment as Mohs surgery. The patient is also notified during this step. Mohs Surgery
Mohs surgery can only be performed on pathology proven lesions, that is, the patient must have pathology proven skin cancer items in the Skin Cancer History Log. This information is added during the office encounter visit when a biopsy is ordered and during the subsequent chart history encounter when the pathology results are documented and added to Skin Cancer History Log. Record Post Procedure Details
On the Intake template, select the Specialty as Dermatology and Visit Type as Mohs Surgery Visit. Click the Procedure tab. The Derm Proc Mohs 1 template opens. Go to the Post Procedure panel. To select whether the Patient tolerated procedure well, select Yes or No option as appropriate. To record complications, select Yes or No. In the Procedure performed today section, select the appropriate option to record the procedure that you performed. Document Mohs Site Details
The Site Details panel enables you to review the site relevant details for the selected finding. You can edit the details and add additional information as required. On the Intake template, select the Specialty as Dermatology and Visit Type as Mohs Surgery Visit. Click the Procedure tab. The Derm Proc Mohs 1 template opens. Go to the Site Details panel, and click the skin cancer record that you are treating using Mohs surgery. The application populates the fields on the Site Details panel with the data from the selected record. To modify any of the disabled fields (those in gray) in the panel, click Edit. The Skin Cancer History Log - Derm window opens and all the fields are available for editing. Modify the required details and click Save & Close. Select the Patient verified site check box. Enter the Mohs # and Path lab fields. The Post op size and cm fields are calculated automatically when the Mohs stage information is added. Parent topic: Access and Document Mohs Surgery Procedure
