Record Neurovascular Exam Findings

The Neurovascular panel is where neurovascular findings are documented, including pulses, sensory distribution, and reflex responses. Links to templates used for documenting detailed neurovascular exam, light touch, and pin prick findings for upper or lower extremity may be found on the panel.
On region specific physical exam templates, the Neurovascular panel may only contain a description statement for upper extremity or lower extremity or it may contain a link to the detailed upper or lower extremity Neurovascular physical exam template.
Neurovascular

  1. If findings are normal for Lower extremity neurovascular normal, select the appropriate check box.
  2. To document that the Lower extremity neurovascular is normal except for something:
    1. Select the Except for check box.
    2. Click the corresponding field and select from the multi-select picklist.
    3. If applicable, select the top blank line to type in the field.
  3. If findings are normal for Upper extremity neurovascular normal, select the appropriate check box.
  4. To document that the Upper extremity neurovascular is normal except for something:
    1. Select the except for checkbox.
    2. Click the corresponding field and select from the multi-select picklist.
    3. If applicable, select the top blank line to type in the field.
  5. In the Pulses section, document pulse measurement findings for locations as appropriate, including right and left sides as applicable.
    1. Click the Right field and select from the list.
    2. Click the Left field and select from the list.
    3. To enter a value not listed, select the top blank line to type in the field.
    4. Repeat for all pulse locations as appropriate.
  6. In the Reflexes section, document reflex measurement findings for locations as appropriate, including right and left sides as applicable.
    1. For right and left sides as applicable, select the option Normal or Abnormal for the Babinski reflex.
    2. Click the Right field and select from the list.
    3. Click the Left field and select from the list.
    4. To enter a value not listed, select the top blank line to type in the field.
    5. Repeat for all reflex locations as appropriate.
  7. In the Sensation section, document sensation exam findings for locations as appropriate, including right and left sides as applicable.
    1. Select the option Normal or Abnormal for right and left sides for the nerves, as appropriate.
    2. Click in the Right field and select from the list.
    3. Click in the Left field and select from the list.
    4. To enter a value not listed, select the top blank line to type in the field.
    5. Repeat for all sensory locations as appropriate.
  8. Click Save & Close.
