Document the Patient's Reason for Visit as a Provider

The reason for visit added by the intake staff on the Intake template appears on the SOAP template. The Introduction field is an editable field that displays a brief summary of the patient's reason for visit. Open NextGen® Enterprise EHR. On the SOAP template, in the Reason for Visit panel, do one of the following: Select one of the reasons for visit entered previously. Select a complaint from the reasons for visits in the left pane. The associated HPI template opens. Note: If you select reason(s) for visit using the Additional / Manage link, then the reasons appear in the Chief Complaint column in the data grid. You must select the reasons listed in the Chief Complaint column in the data grid to open the associated HPI template. Reason for Visit panel On the associated HPI template, enter the appropriate data. Enter any comments. Select Save & Close. The entered details appear in the History of Present Illness column in the Reason for Visit panel. Parent topic: SOAP Template

Reviewing Patient History on SOAP

You can review and edit a patient's medical history in the History panel on the SOAP template. The following categories in the History panel provide the corresponding history details. Past Medical/Surgical Interim Social Family Diagnostic History Panel The Social Determinants of Health and PRAPARE® links appear based on the Practice Configuration practice template settings. Review Patient's History Summary on SOAP You can review and update a patient's history summary documented by the intake staff during the previous encounters, in the History panel on the SOAP template. Adding Patient's History Summary on SOAP You can update the history details entered by the intake staff and add any additional history details in the History panel on the SOAP template. Remove a Diagnostic Study from Reason for Visit using the History Summary Panel

You can remove the studies reviewed record from the Reason for Visit panel using the History Summary panel. You can remove diagnostic studies from the Reason for Visit panel using the History Summary panel. Open NextGen® Enterprise EHR. From the History Summary panel, select Diagnostic. Select Add/Edit. The Diagnostic History Entry template opens. Select a diagnostic study that is previously added to the Reason for Visit panel. The Remove from HPI Grid button is enabled. Select Remove from HPI Grid. The Studies Reviewed row no longer appears on the Reason for Visit panel. Note: Removing the record from the Reason for Visit panel does not remove it from the grids on the Diagnostic History Entry template or the History Summary panel. SOAP Template

You can review and modify the documented patient intake and histories information on the SOAP template. You can review the Asthma Flowsheet, Allergy Test Flowsheet, Spirometry, and Nebulizer panels. You also can document physical exam, assessment plan, and communication details. SOAP Link Icons The icons for various links appear on the SOAP template. Patient's History Summary

You can add, update, delete, and review a patient's history summary details from the History Summary panel. You can document a patient's history information in the History Summary panel in the following categories: Past Medical/Surgical Interim Social Family Diagnostic History Summary The Social Determinants of Health and PRAPARE® links appear based on the settings in the PRAPARE® section of the Practice Configuration template. Social Determinants of Health enables the documentation of social, behavioral, and psychological data for the patient. Review Patient's History Summary on SOAP

You can review and update a patient's history summary documented by the intake staff during the previous encounters, in the History panel on the SOAP template. The various categories available in the History panel enable you to review the corresponding history records. Open NextGen® Enterprise EHR. Open the SOAP template. In History, select a history category in the left pane, and review the history records. For the Social category, the social history details appearing may vary depending on the patient's age. To edit a history record, select the record in the grid, and update the desired details on the category specific history template. Note: For the Social category, you must select the subcategories to review and update the social history details on the Social History template. To delete a history record, select the record in the specific category, and select Remove. Note: You cannot delete the history records in the Social and Diagnostic categories.
