OB Audit Log Report Criteria

OB log reports follow the criteria based on each encounter, as follows: Display a log entry if: The same provider modified content in the same encounter after 24 hours of creating the encounter. The same provider modified content in a different encounter. A different provider modified content all encounters anytime. Do not display log entry if the same provider modified content during the same encounter within 24 hours of creating the encounter. Note: If there is no prior records or changes in Ob Initial Physical Exam template or Auto-calculation of Parity in the OBGYN Synopsis template, the OB Audit Log grid remains blank. Generate OB Audit Log Reports

The OB Audit Log practice template enables you to generate audit logs for a specific time period. The OB Audit Log practice template enables you to generate two different audit log reports: OB PHYSICAL EXAM AUDIT Parity Detail Auto-Calculation These reports capture the changes made to the OB physical exam data based on encounter findings. The OB Audit Log practice template retrieves data from the Ob Initial Physical Exam template. They also show the activation changes made in the Parity auto-calculation activated practice setting. Usually, data for the Ob initial physical exam and parity details should not change for data integrity purposes. Data integrity helps the provider in making decisions about patient care. It is vital that we know not only what data has been captured, but also who is responsible for it. The function of the OB Audit Log practice template is to provide a timeline of all such actions. Tracking Changes of Physical Exam Findings and Parity Auto-calculation Activated Setting

You can keep track of changes of OB Initial physical exam findings and auto-calculated parity values through OB audit log reports. The OB Audit Log practice template enables you to identify what data was changed post the initial documentation, and which can be further reviewed and corrected. You can use this template to: Track changes to the OB Initial Physical Exam Findings template. Track changes to the practice setting Parity Auto-calculation. You can compare the clinical records based on information in the following fields of the OB Initial Physical Exam template: User Name of who changed the record Encounter Id Modified Date/Time What field was altered What the old value was before the change was made What the new value was after the change was made Parent topic: OB Audit Log Practice Template

Generate the OB Audit Log

In the OB Audit Log practice template, click the Type of Log field. A list of audit log types opens. In the Date Range section, click Start and select a start date from the calendar. Click End, and select an end date from the calendar. Click Populate to generate the audit log. A row is added to the Ob audit grid and populated with the following details: User Name Encounter ID Modified Date/Time Field Old Value New Value Note: If no data is present in the OB Initial Physical Exam Findings template, the audit log remains empty and no row is added to the grid below. Document Information in the Prenatal Visits Panel

The Prenatal Visits panel contains a grid with data entered from all previous prenatal visits for the current pregnancy. The comments can be viewed in two ways. Open the Prenatal Visits panel. Select the row with an asterisk in the comments column, and view the full text of the comment in the Progress note field below the grid. Select the Progress Note Summary button. The Pregnancy Note Summary template opens, Prenatal Visits Select the OB Initial Exam link. The OB Initial Physical Exam window opens. Select the Default normal check box. All physical exam items up to the Adnexa item are marked as normal except for the Uterus size item. Any item that is not normal can be manually changed. Uterine size must be manually entered. Other exam items can be entered as needed. Note: If the initial exam is completed at the initial visit, the Lock (complete)initial physical exam check box at the bottom indicating that the exam is complete can be selected.
