Maintain Family Data for the Client

  1. On the Family Information panel of the BH CA Social History template, select No or Yes for Does the patient have children.
  2. If Yes, enter demographic information about the client’s children.
  3. To delete data from the related fields before it is saved to the grid, click Clear.
  4. Click Add to enter the information into the grid.
  5. Document Family history and relationships.
    The Family health history section is available for medical history documentation.
  6. Click Add to enter new data on the Family Health History template.
  7. Click the History Review link to launch the template on which you can determine what prints in the generated documentation:
  8. Record the Pertinent family medical, mental health and substance use history including Adult Child of Alcoholic (ACOA)/Child of Substance Abuser (COSA) status.
  9. To enter additional information, click the Comments field.
