Non-Laterality View of Dynamic HPI

The non-laterality view contains a single grid and a field that fills with the concatenated text. This text appears in the Reason for Visit grid and on the generated documents. When you click Intro, the options available for the introductory text varies based on the chosen Reason. In addition, the sections available in the left pane are different for each Reason. These sections of the HPI enable you to dynamically build the historical note. An asterisk at the beginning of the section indicates that it is added to the concatenation. The selected data also populates in the History of Present Illness column on the shared grid that appears in both the HPI and SOAP templates. The concatenation functionality is set up so that you can skip sections in the left pane that are not needed. There are no rules pertaining to the order of selection or the amount of data that can be included in the history. Enter Comments on Dynamic HPI

You can enter comments or additional information at any time during the documentation of the history of present illness on the dynamic HPI template. On the dynamic HPI template, click the Add/Edit Comments button. The Section field fills with the current section. Click the down arrow next to the Section field to change to a different section, as needed. Type the desired information into the Comments field on the Dynamic HPI Comments template.Dynamic HPI Comments Click Save & Close to return to the dynamic HPI template. Dynamic HPI Views Explained

There are two views of the dynamic template that are based on data (not specialty content): Laterality view - enables documentation of body parts on one side of the body or the other in two separate grids Non-laterality view - enables documentation of body parts in a single grid Both views use the same template shell. The information that appears in the Reason for Visit grid and in the document depends on the data that is selected. The left pane in each dynamic HPI template lists the sections available for the reason for visit. When a section is selected, the findings associated with that section appear in the right grid. The bottom grid concatenates values based on the sections and associated findings documented for the reason for visit as they are selected. Dynamic HPI Templates

In the NextGen® Adaptive Content Engine templates, the following types of History of Present Illness (HPI) templates are used to document a patient's reason for visiting the office: Static HPI - displays hard-coded content that is not easily customizable; a single template is available for each complaint with options for selecting pre-determined details: the final concatenation is available when you close the template Dynamic HPI - enables you to add, edit, update, and delete dynamic content for the HPI and concatenates the content on the template as items are selected or edited; NextGen® Adaptive Content Engine is packaged with pre-configured dynamic HPI templates for the Gastroenterology, Physical Therapy, Orthopedics, Urology, and Pediatrics Well Child; the specific details about these existing HPI elements are provided in the Specialty Specific Dynamic HPI Elements section of this user guide. Document with the Laterality View

Open the Intake template and enter a Specialty and Visit Type.Enter Physical Therapy for the Specialty and Physical therapy evaluation for the Visit Type. In the Reason for Visit panel on the Objective tab, do one of the following to open the corresponding HPI template: If no Reason for Visit was entered during the intake process, select a reason for visit value (for example, elbow) . If a Reason for Visit appears in the grid, click the desired row. In other specialties, the Reason for Visit panel is also available on the SOAP tab. At the top of the dynamic HPI template, a ribbon field appears with any other documented reasons. To change the Reason on the dynamic HPI template, do one of the following: Click the blue arrow next to the Reason field to select a value from the list. In the top ribbon, click the arrow to toggle and select a previously documented reason and move that value to the Reason field. Click intro in the left pane. Each reason includes an intro. Pediatrics Dynamic HPI Views

In the Pediatrics specialty, the dynamic HPI template configured for certain chief complaints are mainly based on the non-laterality view as most patients do not have an issue that pertains to one side of the body or the other. Some chief complaints such as fibromyalgia, juvenile idiopathic arthritis, and med check include the laterality view to document location and the associated symptoms on one side of the body or other.
