On the Procedure - Vasectomy template, you can document the details of the vasectomy procedure including pre-procedure care, anesthesia, procedure findings, and post-procedure and follow-up instructions.
- On the SOAP template, select the Procedures link.
The Procedures template opens.
- In the Genitourinary section, click Vasectomy.
The

- In the Indication section, do the following:
- Click Description, and select the diagnosis.
The code is automatically filled.
- Click Status, and select the diagnosis status from the list.
- To enter a saved default set, click Quick Load and Save.
- In the Pre-Procedure Care section, select the appropriate check boxes after explaining the procedure details to the patient.
Note: To add more information about the consent, click the Consent link.
- In the Anesthetic section, select the anesthesia provided, and then enter the dosage, lot number and expiration date.
- Select the appropriate check boxes and options related to the procedure performed.
- To document the genitourinary exam findings, click GU Exam.
- In the Instructions and Follow Up section, do the following:
- Select the appropriate post-procedure instructions for the patient.
- Enter the follow-up visit instructions.
- Select the medication options.
If you select Yes, enter the medication information in the grid.
- Click Save & Close.
A window opens to confirm if a bill must be submitted for this procedure.
- Click Yes to submit the bill, or No to save and exit.
