On the Circumcision - Procedure template, you can document the details of the circumcision procedure including pre-procedure care, pre-procedure analgesia, anesthesia provided, and devices used to perform circumcision.
- On the SOAP template, select the Procedures link.
The Procedures template opens.
- In the Genitourinary section, click Circumcision.
The
Circumcision - Procedure template opens.
- To enter a saved default set, click Quick Load and Save.
- In the Pre-Procedure Care section, select the appropriate check boxes after explaining the details of the procedure to the patient.
Note: To add more information about the consent, click the Consent link.
- In the Pre-Procedure Analgesia section, select Yes or No for giving analgesia.
- If you select Yes, enter the details of the analgesia given.
- Enter the pain score level.
- In the Anesthesia/Comfort Measures section, select the appropriate options to document the details of anesthesia given to the patient.
- If the procedure was done without anesthesia, select No anesthesia used.
- In the Devices Used to Perform Circumcision section, select the device used for the circumcision and enter the other details of the procedure.
- To document genitourinary exam findings, click GU Exam.
- Select the diagnosis and status of the procedure.
- Enter any additional information in Comments.
- To submit the procedure charges, click Submit to Superbill.
- Click Save & Close.
