The
Menstrual History
panel enables you to document the menstrual history information.
Open the
Gestational Age-Obstetrics
template.
Expand the
Menstrual History
panel.
Select the
Regular
or
Irregular
option in the
Menses
section.
Select the
Frequency
field and enter the number of days in the
Menses frequency
template.
Select the
Menarche onset
field and select a value from the
Menses onset
list.
Note:
This list can be closed and you can manually enter an age.
Select
Yes
or
No
for the
On Oral contraception at conception
section and manually enter in the
Other contraception on conception
field, if needed.
Select a date from the
Positive pregnancy test prior to care
field.

Document General UDS Reporting
The General (UDS Reporting) panel enables you to collect the first prenatal and multiple gestation information. Open the Gestational Age-Obstetrics template. Expand the General (UDS Reporting) panel, if you are required, or if you desire, to collect this information. General (UDS Reporting Select the First, Second, or Third option from the First prenatal visit trimester section. This indicates which trimester the patient initially presented for prenatal care, even if the patient did not present to your practice first. Select the first prenatal visit date from the First prenatal visit field. Select the Here option or Elsewhere option to specify where the first prenatal visit was performed. Select an option in the Multiple gestation section to indicate whether this pregnancy is a singleton pregnancy or not. Documenting the Estimated Date of Delivery Panel
When you open the Gestational Age template, you can view the expanded Estimated Date of Delivery panel appears. Estimated Date of Delivery If a last menstrual period was entered on the Vital Signs template, that last menstrual period (LMP) must display in the LMP Date field. Note: This only happens if the intake staff goes from the Intake template to the Gestational Age template. If they open another template in between, it does not automatically fill the LMP on the Gestational Age template. Therefore, even if the intake staff is not editing any information on Gestational Age, the template must be opened after opening Intake to obtain the LMP from vital signs. Document the Last Menstrual Period You can document the last menstrual period in the Estimated Date of Delivery panel. Select the Most Accurate EDD You can select the most accurate or final EDD in the Estimated Date of Delivery panel. Document the Gravidity/Parity Panel
Before you begin You must enter the gravidity and parity information in the Pregnancy History panel. The Gravidity/Parity panel enables you to view the gravidity and parity information and enter multiple births. Open the Histories template. Expand the Gravidity/Parity panel. Note: You can only view the details in the Gravidity/Parity panel. Gravidity/Parity Optional: To enter multiple births, enter the number in the Multiple births window. Document the OB Histories Template
To document information in the OB Histories template, select the Histories tab. By default, all panels on this template are expanded when the template is opened. Document the Pregnancy History Panel You can document pregnancy history information in the Pregnancy History panel. Document the Gravidity/Parity Panel You can quickly view the patient's gravidity and parity and can only document multiple births information in the Gravidity/Parity panel. Document the Since Last Menstrual Period Panel You can document last menstrual period information in the Since Last Menstrual Period panel. Obstetrics Histories Template
The Histories template contains nine panels: Pregnancy History Gravidity/Parity Since Last Menstrual Period Medical/Infection History Medical/Surgical/Interim General Family Social Genetic Screening/Teratology Counseling OB Histories template The Histories template displays gravidity/parity, initial/working EDD, working EGA, blood type and Rh, and risk level as in Gestational Age template. After a complete history is taken, use the Risk Factors link to document the risk level. Note: When documenting risk factors, the practice may decide to only indicate the positive items rather than select negative for all of the remaining items. In addition, putting the actual risk level must be done by the provider. This may be done on this template or on the Prenatal Detail template. Document the Pregnancy History Panel
You can use the Pregnancy History panel to access the Pregnancy Outcomes template. Note: The Pregnancy Outcomes template is used during Prenatal and Postpartum visits as well as gynecology encounters. The workflow is different for each. In general, the most detail is entered during a Postpartum Visit. Pregnancy History Open the Histories tab. Open the Pregnancy History panel. Select the Add/Update button in the Pregnancy History panel. The Pregnancy Outcomes template opens. If you enable the Parity auto-calculation setting, the Auto-calculation green text appears. Auto-calculation text on Pregnancy Outcomes template If the Parity auto-calculation setting is disabled a red text displays.
