Document the Implant Insertion Details

You can document the implant insertion procedure in the Implant Insertion section on the Contraceptive Implant Procedure template.

1. Open the Contraceptive Implant Procedure template.
2. In the Implant Insertion section, select a diagnosis in Indication for procedure.
   

3. Select a status for the diagnosis.
4. Select the implant type.
5. Enter the lot number and expiration date.
6. Enter the name of the solution used in the skin pepp.
7. Enter the anesthetic and dosage details.
8. In Clinician confirmed placement by palpation, select No or Yes.
9. In Patient confirmed placement by palpation, select No or Yes.
10. Select the location at which the implant has placed as per the manufacturer’s instructions.
11. Optional: In Applied section, select the Adhesive strip (advised remove in 3-5 days) and Pressure dressing (advised remove in 24 hours) check boxes.
12. Enter a date from the calendar in User card given with removal date of.
