You can document the diaphragm or cervical cap fitting details in the Diaphragm/ Cervical Cap Fitting section on the Diaphragm/Cervical Cap - Procedure template.
- Open the Diaphragm/Cervical Cap - Procedure template.
- In the Diaphragm/Cervical Cap Fitting section, select a diagnosis in Indication for procedure.
- Select a diagnosis status in Status.
- Select the Physical Exam link, and then document the physical exam details.
- Select Diaphragm or Cervical Cap.
- Enter the type and size.
- Select the following check boxes as applicable.
- Device fits appropriately
- Patient practiced inserting and removing the contraceptive device in the office
- Care of contraceptive device, use of spermicide jelly, and 6 hour waiting period before removal after intercourse were reviewed
- All patient's questions answered
- Education handout given to patient
