Document the OB Postpartum Visit Template

The OB Postpartum Visit template provides users with an easier way to document a routine OB postpartum visit. The template enables you to document the routine OB postpartum visit without opening other templates. If you need to access other templates an easy access to those additional templates is provided directly from the OB Postpartum Visit template. Wherever possible, the OB Postpartum Visit template fields maps to their corresponding fields in the other templates. The Postpartum template contains the following seven panels: General Reason for Visit Pregnancy Outcome Vital Signs Physical Exam Assessment/Plan Return Appointment Postpartum template Enter Information in the OB Postpartum Visit Template You can enter information in the OB Postpartum Visit template. Reason for Visit on the SOAP Template

Open the SOAP template. Do one of the following: If the Introduction field contains text, review the content and make any necessary changes. If the Introduction field is blank, compose an introduction for the patient. The introduction appears as the first sentence of the visit note, and it can be edited for the individual patient. A typical introduction might contain the patient's age, gender, and the reason for visit. If checked, clear the Do not Launch HPI check box. To add additional details to a previously entered reason for visit, click one of the reasons for visit in the data grid. The appropriate HPI template opens. To add a new reason for visit: Select a complaint from the reasons for visit in the left pane. In the Other field select a reason from the resulting list or type a reason for visit. The associated HPI template opens. On the HPI template, enter the appropriate data. Enter any comments. Click Save & Close. The History of Present Illness column in the Reason for Visit p

Document the Patient's Reason for Visit as a Provider

The reason for visit added by the intake staff on the Intake template appears on the SOAP template. The Introduction field is an editable field that displays a brief summary of the patient's reason for visit. Open NextGen® Enterprise EHR. On the SOAP template, in the Reason for Visit panel, do one of the following: Select one of the reasons for visit entered previously. Select a complaint from the reasons for visits in the left pane. The associated HPI template opens. Note: If you select reason(s) for visit using the Additional / Manage link, then the reasons appear in the Chief Complaint column in the data grid. You must select the reasons listed in the Chief Complaint column in the data grid to open the associated HPI template. Reason for Visit panel On the associated HPI template, enter the appropriate data. Enter any comments. Select Save & Close. OB Postpartum Template

The OB Postpartum template is accessed during a postpartum visit. To document a postpartum visit, select Obstetrics as the specialty and OB - Postpartum or Document Pregnancy Outcome as the visit type. Note: The OB Postpartum visit type can only be used after every pregnancy. If a patient is seen for a wound check after delivery, you must select the Post-Operative Visit. If a patient has just delivered and the practice want to document the details of the delivery immediately, you can select the Document Pregnancy Outcome visit type. The OB Postpartum template includes addition of the Postpartum Details and Pregnancy Outcome panels. The need to compile these data into a single template is necessary for a better workflow. Reason for Visit for Well Child

You can use the Reason for Visit panel on the Pediatric SOAP template to review and update the reasons for visit added during the intake process. The Well child hpi appears on the grid automatically if you select Well child for Visit Type. Reason for Visit In addition to documenting the specialty specific reason for visit available in the left pane, you can also document a well child visit on the a generic HPI template by selecting Well child or Well child HPI in the Reason for Visit panel. In the Reason for Visit panel, you can: Review and update the social history of the patient using the Pediatric Social History link. Open the Interval History- Nutrition template using the Nutrition/Elimination link. Document substance use, psychiatric, placement, sexual practices, and child abuse history of the patient using the Confidential History link. Document the Well Child Reason on the Generic HPI Template You can document the reason for well child visit on the generic HPI template. Document

Document the Well Child Encounter Dynamically

You can dynamically document the reason for visit of a well child using the Well child HPI in the Reason for Visit panel. If the intake staff selects the Well child HPI reason during the intake process, the Well child HPI reason automatically appears on the Reason for Visit panel. If the intake staff does not select the Well child HPI reason, you can select the Well child HPI reason from the left pane. Open the Pediatric SOAP template. In the Reason for Visit panel, click Well child HPI. The HPI template opens. HPI Note: By default, the application selects the well child age group in the Reason field based on the patient's date of birth. To change the well child age group, click the list in the Reason field and select a different well child age group. Document the age-specific complaints about the well child visit.
