Open the Additional Visit Information Config Template
The Additional Visit Information template enables you to collect additional visit information from patients. You can access the Additional Visit Information template from the Additional Screening link in the Pre-Procedure Care section of the Hysteroscopy/Ablation/Sterilization procedure. Open NextGen® Enterprise EHR. From the File menu, select System \ Practice Template. The Select Template window opens. Select the Practice tab. In the Show section, select the All option. A list of practice level templates appears. Double-click Additional Visit Information Config. The Additional Visit Information Config template opens. Add Categories and Questions for Additional Visit Information The Additional Visit Information panel enables you to add categories and questions related to the category. Document Intimate Partner Violence Screening
In the Intimate Partner Violence Screening section of the Family Planning Intake template, you can document intimate partner violence screening. Select a date for the Date of last intimate partner violence screening field. Select the applicable check box(es). If any selections are made, then additional questions appear. Select the Yes or No option for the Have you ever felt pressured to have sexual activities or get pregnant when you didn't want to field. Select Yes or No option for the Has anyone ever threatened or physically hurt you field. If you select Yes, select the applicable option for the Was a report filed field and select a date in the Date report filed field. Document Reproductive Health
On the Family Planning Intake template, in the Reproductive Health section, select the patient's preferred pronoun. The Preferred name field displays the patient's preferred name as recorded in Patient Demographics of NextGen® Enterprise PM. Enter a current birth control method. For Reason for no birth control (at Intake), select a reason. Note: The Reason for no birth control (at Intake) field appears only if the Title X Fields feature is enabled on Practice Configuration. In the Desired Birth Control Method field, select Same as current or Other. If you select Other, select a different birth control method from the additional textbox that appears. For Unprotected intercourse within last 5 days field, select Yes or No. For Unprotected intercourse since LMP field, select Yes or No. To indicate the importance of preventing pregnancy for the next year, select a number from a scale of one to ten. Document Gynecologic History
Click the LMP date in the LMP field. If the LMP was entered on the Vital Signs template, then it appears automatically. Click the Estimated check box. Select a date in the Last pap field. If a Pap smear was entered in the Pap smear grid on the Abnormal Pap HPI or OBGYN Synopsis templates, the system fills in the date. Click the Patient reported check box if applicable. Click the Last mammogram field. The Historical Test – Quick Entry template opens. Historical Test – Quick Entry template Select the applicable option from the Mammogram grid. The Test description and Test code fields display the details of the selected option. Select a date in the Date done field. Entering a date value satisfies the Care Guidelines Mammogram entry. Note: The entered information displays in the Diagnostic Studies grid. If appropriate, enter the interpretation and results in the Interpretation and Results fields. These are optional fields. Document Family Planning Information in MA Intake Sub-Navigational Link
The Family Planning Intake template is for medical assistants to collect and review Family Planning or Reproductive health-related intake data. To include all documented confidential history on the patient document, select the Include all confidential information in the social history document with patient permission check box. You have to configure the MA Intake sub-navigational link. After the MA Intake sub-navigational link is configured, you can access the Family Planning Intake template by clicking on the MA Intake sub-navigational link. Overview of the Histories Template
You can add, update, delete, and review a patient's history summary details. You can document a patient's social history, problem list, medical or surgical history, diagnostic study history, and family history on the Histories template.
