Document Counseling and Education Details

You can document patient's counseling and education details using the Counseling Details template.

1. Open NextGen® Enterprise EHR.
2. Select the Care Guidelines link on any of the main template.
   The Care Guidelines Paged View template opens.
3. In the Care Guidelines panel, select the Grid View link.
   The Care Guidelines grid view template opens.
4. Expand the Nutrition panel, select the Details link.
   The Counseling Details template opens.

   

5. To document counseling or education details, do the following:
   • In the first row, select the field and document each of the following as appropriate, and select a value from the list for:
     • Type of Counseling
     • Method of Counseling
     • Evaluation of Counseling
     • Time - Enter the time spent using the onscreen numeric keypad.
6. Enter any comments in the Comments field.
7. Document any education materials in the Education materials field.
8. If applicable, to document interpreter details, do the following:
   1. Select the corresponding Interpreter used check box to indicate that an interpreter is used.
   2. Select Language from the list. To enter a value, enter text.
   3. Enter name of the interpreter.
   4. Select Relationship from the list. To enter a value, enter text.
9. To document counseling or educational factors details, do the following:

   Select the check boxes corresponding to the items that apply for the following below, and select Other to enter a value.

   • Readiness to learn
   • Barriers to learning
   • Learning preferences
10. To include Counseling/Educational Factors in the visit note for the encounter, select Detailed document. The default option is Reviewed.
11. Select the Cultural/spiritual needs options No or Yes, or enter the text in the text field.
12. Demographic information documented elsewhere will appear for:
    • Marital status
    • Race
    • Religion
13. Select Save & Close.
