Document Counseling Details

You can document counseling details, educational materials provided or used, or interpreter details for a client on the Counseling Details (BH) template. Multiple people in the practice can make entries on this template. From the Evaluation and Management Coding panel on the Finalize (BH) template, select the Counseling Details link. The Counseling Details (BH) template opens. In the Counseling/Education Details section, enter the following for each counseling item: Type of counseling Method of counseling Evaluation of counseling Time The name of the logged-in user fills the Counselor field. Today’s date fills the Date field. Enter the number of minutes spent on the counseling item. Enter Comments. Note: For billing and documentation purposes, the steps described above are the only required fields. Enter Educational materials provided or used. You can document patient's counseling and education details using the Counseling Details template. Open NextGen® Enterprise EHR. Select the Care Guidelines link on any of the main template. The Care Guidelines Paged View template opens. In the Care Guidelines panel, select the Grid View link. The Care Guidelines grid view template opens. Expand the Nutrition panel, select the Details link. The Counseling Details template opens. Counseling Details To document counseling or education details, do the following: In the first row, select the field and document each of the following as appropriate, and select a value from the list for: Type of Counseling Method of Counseling Evaluation of Counseling Time - Enter the time spent using the onscreen numeric keypad. Enter any comments in the Comments field. Document any education materials in the Education materials field. Enter Counseling Details

You can enter the counseling details for a patient and include them in the visit note. Open NextGen® Enterprise EHR. For a patient record, open a new encounter. Open the Finalize template. On Evaluation and Management Coding, select the Counseling Details link. The Counseling Details template opens. Counseling Details template In the Counseling/Educational Details section, do the following: Select the counseling type, method, and evaluation. The counselor's name and date appear. Enter the time spent in patient counseling. Enter additional counseling information. Enter the type of educational materials the patient received. To enter details about an interpreter, select the corresponding check box, and then enter their language, name, and relationship with the patient. You can enter details for up to five types of counseling and education. Total counseling time (minutes) displays the sum of the total time entered for counseling sessions you entered. You can use the Counseling Details template to document the counseling and education details of a patient. Open NextGen® Enterprise EHR. On the Home Page - Cardiology template, click the Counseling Details link. The Counseling Details window opens. Counselling Details In the Counseling/Educational Details section, enter the details of Type of counseling, Method of counseling, Evaluation of counseling, the time spent in the Time field (this calculates the total number of minutes spent counseling the patient during the encounter), Comments, Educational materials, Interpreter used, Language, Interpreter’s name, Relationship to the patient. In Counseling/Educational Factors, indicate if a detailed document or document that the counseling/educational factors were reviewed. In Counseling/Education Factors, select from the following sections. Document a Detailed Ophthalmic Plan

You can document a detailed plan on the Oph - Plan Details template. On the Assessment/Plan - OPH template, in the Plan section, the OPH Plan link opens the Oph - Plan Details template. As you select details from different sections of the template, a summary statement appears in the Plan summary field. oph - plan details template Click the OPH Plan link to open the Oph - Plan Details template. Document Information about Individuals Present during an Assessment

You can document details about the individuals present during the patient's assessment. On the assessment template, expand the Individuals Present panel. Select the contact from Contact Type. Information in POS Description and Code is filled in automatically. Select the reason for contact. Indicate whether the individual is present and, if not, enter the reason for the absence. If other individuals are present, select Others present, and then enter information about those individuals. To enter information about a support resource person, select Add. In the Guardian Info window, do the following: In the Patient Relationship/Support Role section, enter information about the relationship with the foster child. Enter the support person's role, name, and dates for the support period. In the Residence section, enter the primary and secondary addresses. Enter details about the primary and secondary communication and the representation timeframe. Select Save. The relationship details are saved
