SOAP Template

You can review and modify the patient’s reasons for visit, review of systems, and vital signs documented by the intake staff on the SOAP template. You can also review the patient's visit type, alerts, and risk indicators. In addition, you can document the physical examination details, history, and plan information. You can review and modify the following details on the SOAP template. General information Reasons for visit History Review of systems Vital signs Physical examination Assessment and plan Although the you could enter the reason for visit and HPI on the Intake template, there are options available on the SOAP tab that are not available on the Intake tab. Open the Care Team Members Template

The Care Team Members template is accessible through a Care Team link which is available on Intake, Histories, SOAP, Finalize, and Checkout templates. Open a patient encounter and then select a specialty and a visit type. From the Intake template, click the Care Team link. The Care Team Members template opens. The SOAP Template

You can use SOAP to review the patient information gathered during intake and to document the exam, assessment, and plan. On the SOAP template, you can review the patient's visit type, messages, and risk indicators. You can indicate if the patient is new or established for the current encounter on the header of the SOAP template. Open the Transitioning Into Care Template

You can view and manage the summary of care details for a patient by using the Transitioning Into Care template. You can only view the details of summary of care documents that are received or imported from a Health Information Exchange (HIE) for a patient. Open NextGen® Enterprise EHR. On the Intake or SOAP template, select General. Select the Transitioning Into Care link. The Transitioning Into Care template opens. Review Intake Comments on the SOAP Template

Providers can review comments entered by the intake staff using the Intake Comments link. Open NextGen® Enterprise EHR. On the SOAP template, expand Reason for Visit, select Intake Comments. Intake Note Generation

You can generate Intake notes on the Intake template after all the intake activities are complete. You can select Generate Intake Note on the Intake template or Intake Note on the Histories templates. You can create an Intake Note as the written summary of the encounter with the intake staff or medical assistant.
