Enter Assessment and Diagnosis Details

Expand the Assessment/Diagnosis panel. If enabled, click the DSM IV link in the upper, right corner to launch the BH – DSM IV template. Note: The display of the link is set up on the Behavioral Health section of the Practice Configuration system template. In the Behavioral Health Diagnoses section, select any of the available links. In the Diagnostic description required field, click to launch the diagnosis search window. Selecting a description value automatically populates the ICD Code field or vice versa. Add Status, Severity, Differential DX, and Impression. The out-of-the-box Severity picklist does not include Extreme or Persistent. These values can be added to the list in File Maintenance. Enter any required Specifiers by doing one of the following actions: Click the field to type a value. Click the My Phrases link to create a new sentence or part of a sentence. Document Individuals Present

In the Individuals Present panel, select from one of the Contact type values. In the Individuals Present section, do on of the following: Select Individual not present and a Reason. Select Individual present. Select Others present and document their names. Enter the client's closest friends and relatives in the Support Resources section and click Add. Note: The First name and the Effective date are required To set the filter, click Active to view just the current resources or click All to see every resource ever associated with this client. Document Sexual Issues

The field data on the Sexual Issues panel carries forward from encounter to encounter. Open the BH CA Social History template and expand the Sexual Issues panel. Enter the client's Sexual Preference. Confidential Information

You can use the confidential history templates to document a patient's confidential information. You can open the confidential history templates by selecting the Confidential History link in the Social panel on the Histories template. The following links enable you to document the confidential information of the patient: Alcohol/Drug Enables you to open the Confidential - Alcohol/Drug template. Psychiatric Enables you to open the Confidential - Psychiatric template. Abuse/Incarceration Enables you to open the Confidential - Abuse/Incarceration template. Sexual Practices/STI's Enables you to open the Confidential - Sexual Practices/STIs template. Provider Comments Enable you to add any other confidential information. With the patient's permission, you can generate the documented confidential details in the social history document. You must select the Include all confidential information in the social history document with patient permission check box to include the details in the social

Document a Patient’s Confidential Sexual History

You can document the confidential sexual history of patients and their partners on the Confidential - Sexual History template. The documented information on this template is automatically added to the Patient Information and Partner Information grids on the Details panel. Open NextGen® Enterprise EHR. To open the confidential templates, do one of the following: In the Social panel on the Histories template, select the Confidential History link. In the History Summary panel on the Intake template, select the Confidential link. The Confidential - Alcohol/Drug template opens displaying the links to the confidential templates, in the left pane. Select Sexual Practices/STIs and then select the Detailed Sexual History tab. The Confidential - Sexual History template opens.
