Add Assessments to a Common Assessment Page
On the Common Assessments practice template, expand the Manage Common Assessments panel.
- Click Common assessment page name and select an option from the list.
- For ICD Version, select ICD-9 codes or ICD-10 codes.
- Click Item # and select the applicable number.
- The system automatically fills #1 or uses the next available line for existing common assessments in the Item # field.
- Click Item to Add and select an option from the list.
Note: Based on this selection, the fields are changed.
- If you select the Description and code option for individual diagnosis codes, the Diagnosis Code Lookup link appears to add description and an associated code in the common assessment list.
- Click the Diagnosis Code Lookup link to search and select a diagnosis. The selected diagnosis appears in the Description and Code fields.
The Full description field displays the ICD description name in the diagnosis codes library in File Maintenance and cannot be changed.
- Click the Add/Update button.
The description and code are inserted into the next position listed in the Item # field.
- Click the Diagnosis Code Lookup link to search and select a diagnosis. The selected diagnosis appears in the Description and Code fields.
- If you select the Group option for a group of diagnosis codes, the Group name and Link to diagnosis group fields appear.
- Enter the diagnosis Group name to appear on the run-time template.
- Click Link to diagnosis group and select an option from the list to assign the code category to the group. This selection depends on the codes created in the Dx Groups Reference Systems Practice template.
- Click Add/Update.
The group name preceded by a (+) sign appears in the list to indicate that the group includes a list of assessments.
- If you select the Spacer option and click the Add/Update button.
A line of dashes appears to the next position listed in the Item # field. The Spacer option is used to put a space between two separate groups.
- If you select the Subgroup option for additional items, the Subgroup name and Link to diagnosis group fields appear.
- Enter the Subgroup name to appear on the run-time template.
- Click Link to diagnosis group and select an option from the list to assign the code category to the subgroup.
- Click Add/Update.
The subgroup preceded by a (+) sign appears to indicate that the subgroup includes a list of assessments.
- When you have completed the changes, scroll to the bottom of the template, and then click Save & Close to activate the changes in the template.
