Add a Custom Care Plan DB Picklist

  1. In the Custom DB Picklists template top grid, select the Care Plans custom DB picklist.
    The selected DBPicklist Care Plans appears in the DBPicklist Name field.

  2. Click Configure.
    The Care Plans Setup template opens with pre-configured data in the grid.

  3. To add new care plan data, click Add New List Items link.
    The fields above the grid become available for documentation.
  4. Click the Practice ID field and select the practice that uses the care plan data.
  5. In the Care Plan Type field, enter any of the following care plans:
    • Problem
    • Intervention
    • Outcome
    • Goal
  6. In the Problem field, enter a category for the problem.
  7. In the Code Set Type field, enter the code type.
    For example, you can include the code type SNOMED for a care plan goal.
  8. In the Code Value field, enter a code to associate with the problem.
  9. In the Plan Details field, enter a description or phrase to associate with the problem.
  10. Click Add.
    The documented care plan appears in the grid.
  11. Click Close.
    On the run-time Care Plan Data template, you can click the Common Phrases link associated with any of the care plan types to view the configured care plan data.
