Record Care Coordination Interactions
Before you begin Ensure that you have selected the Specialty as Care Management and Visit Type as Case & Consent. On the Documentation template, expand the Care Coordination Interactions panel, and then click Add The Care Coordination Interactions template opens. Fill out all the fields that apply for this interaction. Click Save. Click Clear For Add to enter multiple actions or click Close. Note: Use the Edit and Remove buttons to manage Care Coordination. Care Coordination Home Page
The Care Coordination home page provides a focal point for patient care information gathered from all members of the patient care team and available for review by those team members. Using the home page, you can: Identify the members of the patient care team and define their roles. You can also make changes to the team as needed. The Care Coordination home page is the only location where team members can be defined and the definition modified. When you define an intervention, you must identify a team member who is responsible for that intervention. Enter, review and modify referral information. Identify barriers to care such as patient disabilities and other issues that impact the patient’s ability to comply with a nursing care plan. Review and modify the current nursing care plan and track nursing care plan progress. Log and review all patient-related communications, including phone calls, emails, and text messages. Identify Care Coordination Providers
Before you begin Ensure that you have selected the Specialty and Visit Type as Care Management. All providers, staff, coordinators and so on involved in a patient’s care are indicated in this panel. Update a Referral
Click to highlight the referral on the Referral History grid that you want to update. Click Edit. The Referrals – Orders template appears. Referrals Order Update the template with the new information. Click Save. You are returned to the Care Coordination Home Page with the entry for the referral updated in the Referral History grid. Access the Medication Classification Practice Template
From the File menu, select System\Practice Template. The Select Template template displays. Click the Practice tab. In the Show section, click the All option. A list of practice level templates displays. Select Medication Classifications , and then click OK. The Medication Classifications practice template displays. Alternatively, you can click the Medication Classes link on the Care Guidelines Setup practice template to launch the Medication Classifications practice template.
