Add or Update Assessment Template

The Add or Update Assessment template that launches from the Behavioral Health templates differs from the core specialties. In this version, the template is updated to reflect the standards put forth in DSM-5 by incorporating the Severity and Specifiers into the data collected into a grid, among these other changes: The Site and Axis fields are hidden when the specialty is set to Behavioral Health. The Severity field launches a picklist with the following selections: Mild Moderate Severe Persistent Extreme The grid includes the following headers: # - the priority number of diagnoses entered for Today's Assessment. Access the Add or Update Assessment Template

Select the Add/Update button below the Today’s Assessment grid on the MICS MedAdmin OPH template. The Add or Update Assessment template opens. Diagnoses already attached to treatments for this patient appear in the Billing Diagnosis History grid in the upper left. Note: The Add or Update Assessment template is part of a template set, the templates of which are indicated and accessed by the tabs along the top of the window. Open the Assessments Tab

There are several options for accessing the Add or Update Assessments template: On the SOAP template, in the Assessment / Plan panel, select Add/Update. On the Intake or SOAP templates: Select the My Plan link to open the template set. My Plan Select the Assessments tab. Behavioral Health Common Assessment Page

The BH Assessment page that opens in the Add Common Assessment template includes the following updates for DSM-5: When the Practice Configuration practice template option to display a link for the Diagnostic and Statistical Manual of Mental Disorders Fourth Edition (DSM IV) is not selected, the Add Common Assessment template for Behavioral Health displays an updated grid and picklist (replacing the required, DSM IV, Axes lists). The Status picklist is not required. Users can cancel out of the list without making a selection. Update an Assessment on the SOAP Template

Open NextGen® Enterprise EHR. On the SOAP template, in the Assessment / Plan panel, select an assessment you want to update in the grid. Select Add/Update. Add an Assessment on the SOAP Template

You can enter assessment and plan details on the Add Assessment tabbed template. Open NextGen® Enterprise EHR. From the SOAP template, in the Assessment / Plan panel, select Add/Update. The Add or Update Assessment tabbed template set opens. Select an assessment. Enter the Status, Side, Site, Impression, and Differential dx fields. Select Add/Update.
