Document Interventions, Methods, and Actions

Expand the Intervention(s)/Method(s)/Action(s) panel. In the Linked to goal field, click to select from the list of previously entered goals. Note: Linked to goal, Linked to Objective, and Intervention are required fields in this panel. In the Linked to objective field, select a previously selected objective. Enter the Intervention. In the Service description/modality field, select a type of therapy/group from the list or click on the top, blank line to type a value in the field. Set the Frequency of how often therapy and/or group sessions take place. In the Responsible field, select a type of provider from the Intervention Staff list or choose the top, blank line to free text a value. Select Active or Inactive. Click Add. Once the information has been added to the grid, additional interventions may be recorded as needed. When the IAP or BH Master document is generated, the intervention, method and/or action information appears under the status selections made on the BH - Individualize

Complete the IAP General Panel

On the BH - Individualized Action Plan (IAP) template, click the Program name field to select an item from the list. Note: Program names can be added and/or edited by the practice on the Custom Picklists Setup, system template. Entering a program name helps when setting up reports for the practice but is not required. Set the Admission date. Enter an Effective date of initial IAP. BH - Individualized Action Plan (IAP) Template

The Individualized Action Plan (IAP) template is the master treatment plan of care for the client. Following the Comprehensive Assessment exam, the Behavioral Health provider may choose to initiate an IAP. The IAP is completed in collaboration with the Behavioral Health client and a copy of the finished plan is usually signed by the client. The action plan template enables users to leverage electronic functionality by tying together the data across the client’s assessment, IAP, and Progress Note. Because this information is shared, the user is not repeatedly entering the same data. Individual Visit Workflows for Behavioral Health

Individual visit workflows for Behavioral Health enable the documentation of assessment and treatment plans for the individual client and are associated with the following Visit Types: Home Visit Psychiatric Diagnostic Evaluation Psychiatric Diagnostic Evaluation with Medical Services Psychotherapy - Individual Psychotherapy - Individual with E&M Virtual Visit Telephone Virtual Visit Video You can record new issues for the client on the Individualized Action Plan (IAP) template. The Comp Assess tab can be used to access the Treatment Recommendations/Assessed Needs panel. The IAP tab is used for the initial documentation of the client's goals, objectives and interventions for the assessed needs.
