BH - Brief Assessment Template

The BH- Brief Assessment template enables providers, case workers and social workers, to document on an abridged version of the Comprehensive Assessment template set. Because a comprehensive exam takes a significant amount of time (more than a single client visit), the BH - Brief Assessment template can be used to complete the evaluation during one encounter. The BH - IAP Review/Revision Template

The BH - IAP Revision/Review template is used to document progress or lack thereof and demonstrates evidence and/or rationale for the amendment of treatment goals and objectives in the Individual Action Plan (IAP). The system automatically pulls forward data from the last IAP or IAP review to the current encounter. Comprehensive Assessment Template Set

The workflow of a comprehensive assessment is a set of five templates that enable you to perform a complete intake evaluation for either an adult or pediatric, Behavioral Health clients. The comprehensive assessment is meant to be completed by a clinician designated by the practice. This occurs during the agency’s intake process. From the Comp Assess tab is also known as the BH CA Presenting Concerns template. Document Interventions, Methods, and Actions

Expand the Intervention(s)/Method(s)/Action(s) panel. In the Linked to goal field, click to select from the list of previously entered goals. Note: Linked to goal, Linked to Objective, and Intervention are required fields in this panel. In the Linked to objective field, select a previously selected objective. Enter the Intervention. In the Service description/modality field, select a type of therapy/group from the list or click on the top, blank line to type a value in the field. Set the Frequency of how often therapy and/or group sessions take place. In the Responsible field, select a type of provider from the Intervention Staff list or choose the top, blank line to free text a value. Select Active or Inactive. Click Add. Once the information has been added to the grid, additional interventions may be recorded as needed. When the IAP or BH Master document is generated, the intervention, method and/or action information appears under the status selections made on the BH - Individualize

Intake Visits for Behavioral Health

Intake visits are where the Comprehensive Assessment exam, including a substance use assessment and mental status exam are documented. Users can also choose to document an Individualized Action Plan (IAP), screening or admission note, or a psychiatric evaluation from an intake-based encounter. There are two types of Intake visits: with or without medical services. Intake No Medical Services can be used by anyone at the practice. Intake with Medical Services is designed to be used by providers who may be performing a physical exam and/or prescribing pharmaceuticals. Individual Visit Workflows for Behavioral Health

Individual visit workflows for Behavioral Health enable the documentation of assessment and treatment plans for the individual client and are associated with the following Visit Types: Home Visit Psychiatric Diagnostic Evaluation Psychiatric Diagnostic Evaluation with Medical Services Psychotherapy - Individual Psychotherapy - Individual with E&M Virtual Visit Telephone Virtual Visit Video You can record new issues for the client on the Individualized Action Plan (IAP) template. The Comp Assess tab can be used to access the Treatment Recommendations/Assessed Needs panel. The IAP tab is used for the initial documentation of the client's goals, objectives and interventions for the assessed needs.
