Title X 2.0 Template Overview

The Title X 2.0 template displays all the Title X 2.0 clinical information recorded for a patient during a family planning encounter. You can review the present and previously recorded patient details for Family Planning Annual Report (FPAR) 2.0. The patient’s recorded Title X 2.0 information is categorized into various panels, such as General, Social History, Confidential History, Assessment / Plan Vital Signs, and Testing and Results. The Title X FPAR 2.0 report is required under the Title X Service Grants and is generated by all family planning clinics receiving the Title X grants. The Office of Population Affairs (OPA) requires all Title X grantees to collect and submit FPAR data at the encounter level for the yearly reporting period, starting January 1 through December 31. The Title X FPAR report 2.0 must be set up and licensed for your practice. When the Title X 2.0 software is installed, the lab tests and the results also appear on the Title X 2.0 template. Document Patient Confidential Sexual Practices

You can document a patient’s confidential sexual practices in the social history document. Open NextGen® Enterprise EHR. Select a patient, create an encounter and set the Specialty as Family Practice and Visit Type as Office Visit. To open the confidential templates, do one of the following: In the Social panel on the Histories template, select the Confidential History link. In the History Summary panel on the Intake template, select the Confidential link. The Confidential - Alcohol/Drug template opens displaying the links to the confidential templates. Document the Obstetrics and Gynecology Synopsis Template

The Obstetrics and Gynecology (OBGYN) Synopsis template provides the details of female patients. You can document information on the patient's menopause status, hysterectomy details, gynecological and pregnancy history, abnormal pap history, and contraception information for female patients over the age of eight years. You can use the OBGYN Synopsis template during a gynecology visit, not during an OB prenatal visit. On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN.The OBGYN Synopsis template opens. Select from Primary OBGYN provider. Select from the following options. Confidential Information

You can use the confidential history templates to document a patient's confidential information. You can open the confidential history templates by selecting the following links. The Confidential History link in the Social panel on the Histories template. The Confidential link in the History Summary panel on the Intake template. The links allows you to document the following confidential information of the patient. Alcohol and drug use On the Confidential - Alcohol/Drug template. Psychiatric history On the Confidential - Psychiatric template. Abuse or domestic violence and incarceration On the Confidential - Abuse/Incarceration template. Sexual practices and STI details On the Confidential - Sexual Practices/STIs template. With the patient's permission, you can generate the documented confidential details in the social history document.
