You can document a patient’s confidential sexual practices in the social history document.
 
  - Open NextGen® Enterprise EHR.
 
  - Select a patient, create an encounter and set the Specialty as Family Practice and Visit Type as Office Visit.
 
  - To open the confidential templates, do one of the following:
   
    - In the Social panel on the Histories template, select the Confidential History link.
 
    - In the History Summary panel on the Intake template, select the Confidential link.
 
   
   
    The Confidential - Alcohol/Drug template opens displaying the links to the confidential templates.
   
 
  - Select Sexual Practices/STIs.
   
    The 
Confidential - Sexual Practices/STIs template opens. 
    
    
  - To include the confidential details in the master documents, select the Include all confidential information in the social history document with patient permission checkbox.
 
  - In the Sexual Practices section, do the following:
   
    - Select the Gender identity of the partner.
 
    - For Sexually active, select No, Yes, or Previously.
 
    - Select a sexual orientation and the number of current partners.
 
    - For Practices safe sex, select No, Yes, or Sometimes.
 
    - Select the Details field to select the safer sex details, and then select the number of lifetime partners.
 
    - For Birth control methods (BCM) discussed, select one or two birth control methods.
 
    - For Reason for no birth control (at Intake), select a reason.
     
      Note: The Reason for no birth control (at Intake) field appears only if the Title X Fields feature is configured on Practice Configuration.
      
 
       
    - In the Desired Birth Control Method field, select Same as current or Other.
     
      If you select Other, you must select a different birth control method.
     
 
    - For How important is it for you to prevent pregnancy for the next year, select a number from 0 to 10.
 
    - For Would you like to be pregnant in the next year, select Yes, No, Ambient, or Unsure.
     
      If you select Yes, you must enter a time frame.
     
 
   
 
  - Select OK.
   
    The details are saved and the template is closed.
