Practice Care Team

Practice care teams consist of clinical professionals and agency/organizations who are assembled to provide team care for patients. A practice would typically have multiple care teams defined to address specific needs and you can import the practice care teams to a patient care team. Prior to importing practice care teams to a patient care team, you must configure the practice care teams using the Practice Team Configuration template accessible. When you have configured the practice care team, you can import the practice care team to a patient care team using the Care Team Members template. When you import a practice care team to a patient care team, all the members of the practice care team will become the members of the patient care team in bulk. Remove Patient Care Team Member

Add Patient Care Teams

You can create a patient care team and add participants to the team from the Care Team Members template. You can also add a practice care team as one of the participants of the patient care team. From the Care Team Members template, in the Patient Care Team section, enter a care team name. Select a care team status. By default, the status of the care team is Active. Set the active and inactive date and time of the practice care team. By default, the active date and time is the current date and time. To add a participant to the care team, select a participant type and then do one of the following: In Clinical professional or Agency/Organizations, select a member from the NPI Registry lookup and then add/edit the participant details in the corresponding fields or import a practice care team. Select Other and enter the corresponding details. Note: You cannot add/edit the address details for the Clinical professional or Agency/Organization participant type. Filter Patient Care Teams and Members

Open the Care Team Members template. From the Filter by team name list, select a team. The patient care team and member details appear in the Care Team Details grid. To filter member details based on the status of the care team member, select the status from the Filter by status list. The member details appear in the Individual Care Team Members Details grid. Update Practice Care Team

You can update status, active details, inactive details, and member details of a practice care team from the Care Team Configuration template. From the Care Team Configuration template, select a team from the Practice Care Team Details grid.The details of care team and its members appear in the corresponding fields. To update the care team details, edit the status and/or active and inactive dates. To update the care team member details, edit the status and/or active and inactive dates, and role or function. Patient Care Team Information

The Care Team Members template consolidates information on all patient care teams defined for a patient. The patient care team can include clinical professionals, clinical pharmacists, agencies, organizations, relatives who act as care givers, and any other individual involved directly or indirectly with the care of the patient. This consolidated information provides a comprehensive and up-to-date list of a patient’s care team members and meets Stage 3 Quality Reporting Certification for 2017. The list includes patient care team members that have been entered throughout NextGen® Adaptive Content Engine and also the members that have been added using the NPI Registry Search window. However, members added using the Care Team Members template only appear on the Care Team Members template and are shared with the Patient Care Team panel on the Care Plan template.
