Treatment Log Template

The Treatment Log - Physical Therapy template enables providers to review the patient’s encounter diagnoses, treating assessments, and a history of treatments in a flowsheet format, organized by encounter date. In addition, documents for orders, work notes and patient encounters can be generated here. The Diagnosis panel on the Treatment Log template is read-only. The Diagnosis panel displays the Encounter Diagnosis and Treating Assessment sections. The Treatment Log panel displays a read-only view of treatments by encounter. Physical Therapy Treatment Panel

The Treatment panel enables you to document treatments performed during an encounter. Information documented here is also used for billing purposes. The Treatment grid displays details about the treatment plans. The fields in the Performed Today section below the grid function as a worksheet for documentation of treatment activities performed. Note: Only those treatments detailed in a row with the Performed checkbox selected are billed. Physical Therapy Home Page Template Details

The practice can customize the panels displayed on the Home Page - Physical Therapy template. The following table describes the panels on the template. Panel Description Diagnosis Lists all the assessments to be included in orders and billing. You can view billing assessments for the current encounter, including ICD codes. Goals Displays patient's therapy goals documented by the provider in the Therapy Goals template. Treatment History Displays the treatment history details related to the patient. Historical PE Displays the Historical PE details. Treatment Log Displays the patient's treatment details by encounter. Lists the patient's encounter diagnosis, treating assessments, and a history of treatments in a flowsheet format, organized by the encounter date. Allergies Lists the allergies associated with a patient. Medications Lists the patient's current medications. PT/OT Treatment Template Panel: Treatment Grid Order

On the Treatment template, based on the Visit Type selected, you can choose the position of the Therapeutic Exercise, Therapeutic Activity and Neuro Re-Education grids. In the Filter field, select the visit type you want to rearrange the grids for, and then use the up and down arrows below the grid to adjust the Grid Position. Best practice is to set all PT visit types to have the same grid positioning, and all OT visit types to have the same grid positioning. General Physical Therapy Workflow

The Physical Therapy tabbed template set presents a general workflow for documenting the patient history, physical therapy orders, physical exam, goals, and treatment for physical therapy encounters. Patient's Summary Review for Physical Therapy by the Intake Staff If required, the intake staff can review the patient's historical information documented in the previous encounters on the Home Page - Physical Therapy template. Access Home Page - Physical Therapy You can use the Templates module to access the Home Page - Physical Therapy template. Adding and Reviewing Allergies You can add and review the allergy details associated with a patient in the Allergies panel.
