Calculate an E&M Code under 2021 Coding Guidelines on the Cardiology Finalize Template

To calculate an E&M code under the 2021 Coding Guidelines, you must select the Medical Decision Making (MDM) level or enter time on the EM 2021 Guidelines template that opens from the Cardiology Finalize template. To compare E&M codes, you can switch between the calculation methods. However, you cannot combine codes from both methods. The application uses the last calculated code. Open the Cardiology Finalize template and then expand the Evaluation and Management Coding panel. In the Medical Decision Making or Counseling section, do one of the following: Review prepopulated or select the level of complexity associated with the encounter. Enter the total visit time. To calculate the E&M code based on the MDM level, review prepopulated or select the level of complexity associated with the encounter in the Medical Decision Making or Counceling section. Time Information in Master, Specialty Im, and Other Documents

Based on your practice configuration for offline generation or the document configuration for a submitted code, the application creates the Master Im or Specialty Master document or another document. If the submitted code is calculated based on the total time, the generated document includes the total time in minutes, which includes face-to-face time and non-face-to-face time spent with the physician and/or other qualified healthcare professionals. Submit E&M Codes under 2021 Coding Guidelines on the Cardiology Finalize Template

Before you begin To submit an E&M code, you must first select a diagnosis. For more information on diagnoses, go to NextGen Healthcare Success Community, and download the latest Cardiology User Guide for NextGen® Adaptive Content Engine. Open the Cardiology Finalize template and then expand the Evaluation and Management Coding panel. Evaluation and Management Coding Review the patient's medications and diagnoses. To add or update the medication, click Add/Update. At least one diagnosis must be documented to submit the Evaluation and Management (E&M) code. To enter the counselling and educational details and factors, click Counsel Details. The Counseling Details template opens. If you counsel a patient on tobacco cessation for longer than three minutes, click Tobacco Cessation, enter the relevant information in the Tobacco Cessation template, and then click Save & Close. Note: If you spend longer than three minutes on tobacco cessation counseling, the application submits a separate bill

Based on your practice configuration for offline generation for a submitted code, the application generates an ophthalmology chart note. You can also manually create the chart note. If the submitted code is calculated based on the total time, the generated document includes the total time in minutes, which includes face-to-face time and non-face-to-face time spent with the physician and/or other qualified healthcare professionals. Document Counseling and Education Details

You can use the Counseling Details template to document the counseling and education details of a patient. Open NextGen® Enterprise EHR. On the Home Page - Cardiology template, click the Counseling Details link.The Counseling Details window opens. Counselling Details In the Counseling/Educational Details section, enter the details of Type of counseling, Method of counseling, Evaluation of counseling, the time spent in the Time field (this calculates the total number of minutes spent counseling the patient during the encounter), Comments, Educational materials, Interpreter used, Language, Interpreter’s name, Relationship to the patient. In Counseling/Educational Factors, indicate if a detailed document or document that the counseling/educational factors were reviewed. In Counseling/Education Factors, select from the following sections. Cardiology Provider Workflow on the Finalize Template

The Finalize template allows you to document various patient findings. You can use the Finalize - Cardiology template to do the following. Document assessments, medical decision making, counseling details, and Evaluation and Management (EM or E&M) coding. Submit codes for billing. Open a patient support case, visit document, and referral thank-you letter. Evaluation and Management Coding You can use three types of guidelines to calculate the Evaluation and Management code for Cardiology. Generate Documents You can generate and print various patient documents such as patient support cases, visit documents, referral thank-you letters, and provider communications on the Finalize - Cardiology template.
