To select a different provider name, click the Provider field.
In the Document Type field, click to select a value from the resulting list.
Select the Consult Note or Referral Letter document type to add personalized information in the Opening statement and Closing statement fields.
Select the preferred elements to display in the generated document.
Select the Past Medical History, Family History, and Social History checkboxes to include the details entered for these elements on the templates in the generated documents.
Note: The Social History Confidential sections are not included in any document.
Select from the options to load defaults for all documents or just the selected document type.
Note: Selecting a load default automatically saves the document type or types in the Configured Document Types grid.
If you did not select a load default, click Add to include the provider-level preferences to the Configured Document Types grid.
