Reviewing Assessment and Plan History

You can view the assessment and plan details of a patient in the Assessments / Plans panel, based on the filters selected on the Pre Visit Planning template. If more than one encounter exist for the patient, the Previous Visit and Next Visit buttons are available to open the assessment and plan details for each encounter on an individual page. The View Encounter field displays the encounter date and assigned specialty for the details that appear in the grid row. If documents are not attached to the encounter, the View Document button is disabled. If a document is attached, you can open the documents associated with the filtered encounter. Documenting a Patient's History Summary

You can add, update, delete, and review a patient's history summary details from the History Summary panel. You can document a patient's history details in the History Summary panel in the following categories: Past Medical/Surgical Interim Social Family Diagnostic The History Summary panel also displays the patient history documented by the intake staff on the Histories template. You can add diagnostic studies with a documented Result or Interpretation to the grid in the Reason for Visit panel by selecting the Add to HPI Grid button. The grid includes a list of categories on the left pane to select and view the documented data. You can filter the patient history records by selecting the Medical History or Surgical History option for Past Medical/Surgical. You can view and enter confidential details provided by the patient during the course of the encounter using The Confidential link. Document the History Review

You can document the review details of a patient's history in the generated encounter document. You can also document the review status of medical, surgical, family, and social histories on the template. Open NextGen® Enterprise EHR. On the Histories template, select the History Review link. History Review The History Review template opens. In Med/Surg/Interim Hx, Family, and Social, select the applicable option. Detailed document enables you to view all the details entered in the Histories template in the encounter document. Reviewed, updated enables you to indicate that you have reviewed and updated the patient's histories. Reviewed, no changes enables you to indicate that you have reviewed the patient's histories but made no changes. History unobtainable enables you to indicate that the information is not obtainable and enter the reason in the corresponding field. Select Save & Close. Update Past Medical History

You can update the past medical history for a patient in the Edit Past Medical History template. Open NextGen® Enterprise EHR. Open the Histories template. In the Medical/Surgical/Interim panel, select the medical or surgical record from the grid that you want to update, and select Edit.The Edit Past Medical History template opens. Edit Past Medical History Note: You can select an entry in the Past Medical History grid and can select Edit to open the Edit Past Medical History template. Update the necessary details. Add Past Medical, Family and Social History in Brief Assessment

All of the grids available through the navigation pane on the Histories Summary panel share data and copy forward to the next encounter for the current patient. In the History Summary panel, to indicate what detailed history information prints in the generated document, click the History Review button to open the Histories template. If there is no relevant history, select No relevant past medical/surgical history. Click the Confidential link to record private information provided by the patient during the course of encounter. Note: For an adult patient, the Confidential template set opens, for younger patients, the Peds Confidential History template is launched. The grid defaults to the Past Medical/Surgical view (indicated by the black arrow icon in the navigation pane).
