Document the History Review

You can document the review details of a patient's history in the generated encounter document. You can also document the review status of medical, surgical, family, and social histories on the template. Open NextGen® Enterprise EHR. On the Histories template, select the History Review link.The History Review template opens. In Med/Surg/Interim Hx, Family, and Social, select the applicable option. Detailed document allows you to view all the details entered in the Histories template in the encounter document. Reviewed, updated allows you to indicate that you have reviewed and updated the patient's histories. Reviewed, no changes allows you to indicate that you have reviewed the patient's histories but made no changes. History unobtainable allows you to indicate that the information is not obtainable and enter the reason in the corresponding field. Select Save & Close. Note: Each reviewed area (Med/Surg, Family, and Social Histories) contributes to the Evaluation and Management coding.

Patient's Adult Social History

You can view and document the social history of an adult patient. You can document the social history information of the patient in a one-page panel format on the Adult Social History template. This template does not appear by default. Adult Social History The Social History Review section enables you to document the review status. You can select an option to complete the social history review. Include Historical Reviews in Encounter Documents

The History Review link enables you to include detailed information in the generated encounter document. You can also document the review status of medical, surgical, family, and social histories on the template. History Review Click any of the following options next to the Med/Surg/Interim Hx, Family, and Social labels to determine what prints on the encounter document: Detailed document - displays all the details entered on the history template on the encounter document. Reviewed, updated - to indicate you have reviewed and updated the patient's historical data. Reviewed, no changes - to indicate you have reviewed the patient's history and made no changes. History unobtainable - indicates that the patient's history is not obtainable and enter the reason in the corresponding field. Click Save & Close. Review Patient's History Summary on SOAP

You can review and update a patient's history summary documented by the intake staff during the previous encounters, in the History panel on the SOAP template. The various categories available in the History panel enable you to review the corresponding history records. Open NextGen® Enterprise EHR. Open the SOAP template. In History, select a history category in the left pane, and review the history records. For the Social category, the social history details appearing may vary depending on the patient's age. To edit a history record, select the record in the grid, and update the desired details on the category specific history template. Note: For the Social category, you must select the subcategories to review and update the social history details on the Social History template. To delete a history record, select the record in the specific category, and select Remove. Note: You cannot delete the history records in the Social and Diagnostic categories. Documenting a Patient's History Summary

You can add, update, delete, and review a patient's history summary details from the History Summary panel. You can document a patient's history details in the History Summary panel in the following categories: Past Medical/Surgical Interim Social Family Diagnostic The History Summary panel also displays the patient history documented by the intake staff on the Histories template. You can add diagnostic studies with a documented Result or Interpretation to the grid in the Reason for Visit panel by selecting the Add to HPI Grid button. The grid includes a list of categories on the left pane to select and view the documented data. Patient's History Summary

You can add, update, delete, and review a patient's history summary details from the History Summary panel. You can document a patient's history information in the History Summary panel in the following categories: Past Medical/Surgical Interim Social Family Diagnostic History Summary The Social Determinants of Health and PRAPARE® links appear based on the settings in the PRAPARE® section of the Practice Configuration template. Social Determinants of Health allows the documentation of social, behavioral, and psychological data for the patient.
