Generate Notes Associated with Encounters

You can generate notes associated with an encounter on the SOAP template. Open NextGen® Enterprise EHR. On the Intake or SOAP template, select Generate Notes in the Navigation Buttons section. Select the note to generate from the available list. Select OK. Reviewing Patient History on SOAP

You can review and edit a patient's medical history in the History panel on the SOAP template. The following categories in the History panel provide the corresponding history details. Past Medical/Surgical Interim Social Family Diagnostic History Panel The Social Determinants of Health and PRAPARE® links appear based on the Practice Configuration practice template settings. Review of SOAP Details by Provider

You can review and modify the patient’s reasons for visit, review of systems, and vital signs documented by the intake staff on the SOAP template. You can also review the patient's visit type, alerts, and risk indicators. In addition, you can document the physical examination details, assessment plans, and provider communications. You can review and modify the following details on the SOAP template. General information Reasons for visit History Review of systems Vital signs Physical examination Assessment and plan Narrative notes You can select the Record contains substance use disorder information checkbox on the SOAP template header to include the long or short form of disclosure for Substance Abuse and Mental Health Services Administration (SAMHSA) 42 CFR Part 2 provisions in the generated documents and reports. Document the Patient's Reason for Visit as a Provider

The reason for visit added by the intake staff on the Intake template appears on the SOAP template. The Introduction field is an editable field that displays a brief summary of the patient's reason for visit. Open NextGen® Enterprise EHR. On the SOAP template, in the Reason for Visit panel, do one of the following: Select one of the reasons for visit entered previously. Select a complaint from the reasons for visits in the left pane. The associated HPI template opens. Note: If you select reason(s) for visit using the Additional / Manage link, then the reasons appear in the Chief Complaint column in the data grid. You must select the reasons listed in the Chief Complaint column in the data grid to open the associated HPI template. Reason for Visit panel On the associated HPI template, enter the appropriate data. Enter any comments. Select Save & Close. Send SOAP Notes in NextGen® Ambient Assist

You can use NextGen® Ambient Assist to generate and edit Subjective, Objective, Assessment, and Plan (SOAP) notes from your patient visit, and then send them to NextGen® Enterprise EHR. Note: NextGen® Ambient Assist replaces any existing SOAP note narrative text. To prevent blocking of ambient services, you can add api.nabla.com to your internet allowlist. You can continue with the ambient recording when you switch from wireless to cellular network or the other way around. During an ambient session, if the internet connection is lost, audio can buffer for up to two minutes and can resume with normal transcription when the connection is restored. Ambient is available by default for NextGen® Mobile Pro users. You can request and obtain complimentary NextGen® Ambient Assist trials. For unlimited NextGen® Ambient Assist and other clinical documentation workflows, you must upgrade to the NextGen® Mobile Pro package.
