Record Who Supplied the Clinical Data and Provide an Interpretive Summary

The Clinical Formulation/Interpretative Summary panel is used to record who supplied the information gathered and to enter an interpretative summary or formulation that would indicate areas of need, predisposing, precipitating, or perpetuating factors, and the skills/resources needed to address these areas.

Select as many options as apply under This clinical summary is based upon information provided by.
For each selection, enter the following:
- Name
- Relationship
- How was information provided

Interpretation or Summary

Use the Interpretation/Summary field like a Comments field to record things such as observations, additional information, instructions, and plans. You can type text into the field or use saved phrases from a “My Phrases” set. See Add Comments. The Review Comments link above the field opens a window where you can review comments entered in all sections of the patient’s chart. You can also review and copy comments that were captured on previous encounters with that patient. For more information, see Review, Modify, and Copy Comments. Medical Chart Summary Review by Provider

You can select and view the data documented by the Intake staff in the Medical Chart Summary panel. The Medical Chart Summary panel displays the details documented by the Intake staff and provider in an encounter. The details from the previous encounters also appear in this panel. The data grid includes a list of categories in the left pane. Some of the categories include filters in the grid, which allows you to filter and review the required data. Patient's General Information

The General panel, on the Intake template displays if the encounter is for a new or established patient. The Established patient option is selected by default if the patient has previous encounters. You can document the person providing the patient information in the Historian field. When someone else interprets the patient’s communication, you can use the Interpreter Details link to document the details for Interpreter Details category. You can use the Configurable Questionnaires link to document the details for categories available in the Selected Category picklist. The Transitioning into Care link opens the Transitioning Into Care template that captures electronic Summary of Care requests and/or receipts that are completed electronically and document the information as reportable fields on the template. Patient's History Summary Review by Provider

You can review patient's history summary using the History Summary panel on the Home Page template. The History Summary panel displays the patient history documented by the intake staff and provider on the Intake, Histories, and SOAP templates. The data grid includes a list of categories in the left pane that allows you to view and document the associated data. Chart Summary

The Chart Summary template displays the details documented by the intake/clinical support staff and provider in the framework template set of the Procedure and Procedure Preadmission visit types. The left pane includes a list of categories to select and view the documented data. Some of the categories include filters at the top of the grid, allowing you to filter and review the required data. The data in the grid is read-only. The View Previous button is used to add data from the previous encounters to the current dynamic template. The Quick Save button is used to save the documented data of your current template/panel as a default set, allowing you to use it again when documenting on the same template/panel during future patient encounters. Launch Reconciliation from the Medical Summary Utility

To perform a clinical reconciliation from the Medical Summary Utility, you must first import a Continuity of Care Document (CCD) or Consolidated Clinical Document Architecture (C-CDA) document or have completed a patient medication history request. For details about the Medical Summary Utility, go to NextGen Healthcare Success Community and download the latest Medical Summary Utility Guide forNextGen®Enterprise EHR. Note: The Data Share module is now the preferred tool for initiating clinical data imports and exports. The Medical Summary Utility is subject to deimplementation in a future release. Select a patient if one is not selected. From the Data menu, select Reconciliation Module. If you are not using the Single Sign On feature, the Location window appears. Please Select a Location Window Select a Location and continue. The Clinical Reconciliation window appears. You can now select documents to view and reconcile clinical data.
