General Physical Therapy Workflow

The Physical Therapy tabbed template set presents a general workflow for documenting the patient history, physical therapy orders, physical exam, goals, and treatment for physical therapy encounters. Patient's Summary Review for Physical Therapy by the Intake Staff If required, the intake staff can review the patient's historical information documented in the previous encounters on the Home Page - Physical Therapy template. Access Home Page - Physical Therapy You can use the Templates module to access the Home Page - Physical Therapy template. Adding and Reviewing Allergies You can add and review the allergy details associated with a patient in the Allergies panel. Configuration Options

Review the configuration options and decide what you need to implement to enable you to complete the workflows outlined here. You can configure the following practice templates for the Physical Therapy specialty: Physical Therapy Goal Administration: Enables you to edit or add new existing physical therapy goals. Practice Configuration: Enables you to enter expected treatment times by visit type. In addition, you have options for applying billing modifiers. The ability to use the third-party service provided by Physiotec directly from within the Physical Therapy template set also needs to be configured before using the templates. Treatment Log Template

The Treatment Log - Physical Therapy template enables providers to review the patient’s encounter diagnoses, treating assessments, and a history of treatments in a flowsheet format, organized by encounter date. In addition, documents for orders, work notes and patient encounters can be generated here. The Diagnosis panel on the Treatment Log template is read-only. The Diagnosis panel displays the Encounter Diagnosis and Treating Assessment sections. The Treatment Log panel displays a read-only view of treatments by encounter. Histories Template

Patient history information is documented on the Histories template for a new patient, and reviewed and updated for an existing patient. Histories Patient's Problem List You can manage a patient's problems. Patient's Medical History The Medical/Surgical/Interim panel allows you to document the medical or surgical history of the patient. Patient's Diagnostic Tests and Results You can document a patient's diagnostic tests and the associated results. Patient's Family History You can document the family history of the patient in the Family panel on the Histories template. Objective Template

The Objective template is used by the provider to review and update previously documented information about the patient such as surgical procedure dates, reason for visit, History of Present Illness (HPI), and vital signs. The provider then proceeds to document detailed information for the encounter, such as a physical exam, assessment and plan, observations, goals, treatment, edema, and wounds and incision care. Therapy Goals

The Therapy Goals sub-navigational link launches the Therapy Goals template where goals are documented. Documenting patient goals has been simplified by automating the population of goals in the template based on information documented for reason for visit, History of Present Illness (HPI), and physical exam for the patient. Examples of data that populate in the grid include observation, strength, and pain. Physical Therapy goals are packaged with the system and can be configured on the PT Goal Administration practice template.
