Document the Patient's Reason for Visit as a Provider

The reason for visit added by the intake staff on the Intake template appears on the SOAP template. The Introduction field is an editable field that displays a brief summary of the patient's reason for visit. Open NextGen® Enterprise EHR. On the SOAP template, in the Reason for Visit panel, do one of the following: Select one of the reasons for visit entered previously. Select a complaint from the reasons for visits in the left pane. The associated HPI template opens. Note: If you select reason(s) for visit using the Additional / Manage link, then the reasons appear in the Chief Complaint column in the data grid. You must select the reasons listed in the Chief Complaint column in the data grid to open the associated HPI template. Reason for Visit panel On the associated HPI template, enter the appropriate data. Enter any comments. Select Save & Close. The entered details appear in the History of Present Illness column in the Reason for Visit panel. Parent topic: SOAP Template

Review Narratives on SOAP

With the support of NextGen® Ambient Assist, the ambient-generated narrative text appears automatically in the Narrative Notes panel on the SOAP template. The narrative fields contain information documented through applications, such as NextGen® Mobile and NextGen® Ambient Assist. Open NextGen® Enterprise EHR. Open the SOAP template. In the Narrative Notes panel, review and update the ambient-generated narrative text as required. Note: You can document your narratives using My Phrases. To copy mobile dictations from the previous encounters, select Copy Dictation and use the former mobile dictations from the Mobile Dictation Copyforward template. Reviewing Patient History on SOAP

You can review and edit a patient's medical history in the History panel on the SOAP template. The following categories in the History panel provide the corresponding history details. Past Medical/Surgical Interim Social Family Diagnostic History Panel The Social Determinants of Health and PRAPARE® links appear based on the Practice Configuration practice template settings. SOAP Template

You can review and modify the documented patient intake and histories information on the SOAP template. You can review the Asthma Flowsheet, Allergy Test Flowsheet, Spirometry, and Nebulizer panels. You also can document physical exam, assessment plan, and communication details. SOAP Link Icons The icons for various links appear on the SOAP template. Document the Patient's Reason for Visit as a Provider The reason for visit added by the intake staff on the Intake template appears on the SOAP template. Patient's History Summary Documentation by Provider You can document a patient's past medical or surgical history, interim, social, family, and diagnostic study history in the History Summary panel. Remove a Diagnostic Study from Reason

You can remove the studies reviewed record from the Reason for Visit panel using the History Summary panel. You can remove diagnostic studies from the Reason for Visit panel using the History Summary panel. Open NextGen® Enterprise EHR. From the History Summary panel, select Diagnostic. Select Add/Edit. The Diagnostic History Entry template opens. Select a diagnostic study that is previously added to the Reason for Visit panel. The Remove from HPI Grid button is enabled. Select Remove from HPI Grid. The Studies Reviewed row no longer appears on the Reason for Visit panel. Note: Removing the record from the Reason for Visit panel does not remove it from the grids on the Diagnostic History Entry template or the History Summary panel. Remove a Studies Reviewed on SOAP Template

You can remove the historical diagnostic studies that are added to the history of patient illness in the Studies Reviewed section on the SOAP template. Open NextGen® Enterprise EHR. Open the SOAP template. In the Reason for Visit panel, select the historical diagnostic study item with the Studies Reviewed label in the Reason for Visit grid. Select Remove.
