Patient's Medical History

The Medical/Surgical/Interim panel allows you to document the medical or surgical history of the patient. If no medical or surgical history exists, you can select the No relevant past medical/surgical history checkbox. You can filter the past history records by selecting the required checkboxes. By default, the Show All checkbox is selected. Configuring Medical, Surgical and Family History

This document provides information on how to configure the specialty-specific medical history, surgical history, and family health history content. The Configure Medical, Surgical, Family Hx system template enables you to configure a list of medical, surgical and family health conditions, and management items that are associated with the SNOMED (Systematized Nomenclature of Medicine) codes. In run time, the configured conditions and management items appear in the Past Medical History and Family Health History templates. In the Configure Medical, Surgical, Family Hx system template, you can configure different types of conditions as described in the table. Individual and Grouped Conditions and Procedures

Conditions of interest to a specialty appear on the Medical panel of the Past Medical History template and on the Family Health History template. Both templates enable you to document the patient's medical conditions and their family members medical history. Individual Conditions- Conditions listed without a plus sign [+] to the left of the item represent a single general disease or disorder. Grouped Conditions- Conditions listed with a plus sign [+] are types or groups that represent a category of diseases or disorders, with more than one associated disease or disorder as items to select from. On the Surgical panel of the Past Medical History template, surgery types or groups are similarly organized and identified with a [+], and items listed without a [+] are single surgical procedures. Note: Conditions or surgical procedures with an asterisk (*), indicate that the condition or procedure does not have a SNOMED code. Family Health History Conditions

The Family History panel enables you to select the applicable family health history conditions for a specific specialty and/or practice. The family health history conditions are configured in the Condition and Management Configuration panel. For information on how to configure conditions, see Condition and Management SNOMED Codes. Document a New Family History

You can add the family history of a patient from the Family category in the Family panel on the Histories template. Open NextGen® Enterprise EHR. Open the Histories template and then expand the Family panel. In the Family panel, select Add. The Family Health History template opens. Note: NextGen® Enterprise EHR fills Specialty field with the current specialty. The available conditions for the specialty appear alongside a checkbox on the Family Health History template. To indicate that the patient does not have any family history, select the No family history of checkbox. In the Relationship field, select a description from the Family relation list. In the Family member name field, enter the name of the selected family member, and do one of the following: If the family member does not have any condition that can be reported, select the Alive and well option.
