Document the Pregnancy Outcomes Template

You can document information in the Pregnancy Outcomes template. The Pregnancy Outcomes template provides you the ability to view and modify a patient’s gravidity and parity. You can view and modify the total parity values with or without the details of each pregnancy. Minimal required fields are Pregnancy Entry Type, Pregnancy #, and Delivery Type. It is preferable, on an OB initial visit to use the gravidity/parity fields and Pregnancy Outcomes templates from within the OB templates. When you activate parity auto-calculation, parity automatically calculates the details when you document historical pregnancy or end a current pregnancy When you select End current pregnancy the Gravidity reflects a non-pregnant status. This option is used on a postpartum or post-abortion visit and not on an initial prenatal visit. On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN.The OBGYN Synopsis template opens. Select the Add/Update Detail button in the OBGYN Synopsis template.

Documenting Pregnancy Outcomes

You can document pregnancy outcomes in the OBGYN Synopsis and Pregnancy Outcomes templates. You can document the Pregnancy outcomes in the OBGYN Synopsis and Pregnancy Outcomes templates. Note: In a Gynecology patient, the OBGYN Details link would not be typically used in a pregnant patient. There are two ways to enter new pregnancy: Quick entry Detailed entry Quick Entry of Pregnancy Outcomes You can enter a quick parity entry in the Pregnancy Outcomes template. Detailed Entry of Pregnancy Outcomes You can enter a detailed parity entry in the Pregnancy Outcomes template. Document the Obstetrics and Gynecology Synopsis Template

The Obstetrics and Gynecology (OBGYN) Synopsis template provides the details of female patients. You can document information on the patient's menopause status, hysterectomy details, gynecological and pregnancy history, abnormal pap history, and contraception information for female patients over the age of eight years. You can use the OBGYN Synopsis template during a gynecology visit, not during an OB prenatal visit. On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN.The OBGYN Synopsis template opens. Select from Primary OBGYN provider. Select from the following options. Detailed document: Select this to include the gynecological and pregnancy histories in the document. Enter a Historic Pregnancy in the Pregnancy Outcomes Template

You can enter a historic pregnancy in the Pregnancy Outcomes template. On the Patient Information Card in NextGen® Enterprise EHR, select OBGYN.The OBGYN Synopsis template opens. Select the Add/Update Detail button in the OBGYN Synopsis template.The Pregnancy Outcomes template opens. If you turn on the Parity automatic calculation setting, the Auto-calculation green text appears. Pregnancy Outcomes Select the Historic entry option in Pregnancy Entry Type field. Select the pregnancy number in the Pregnancy # field. Select a Full term or Premature pregnancy in the Gestational age section. For Full term pregnancy, if a pregnancy is >=37 weeks (37 weeks 0 days or later) the Full term field is incremented by 1. For Preterm pregnancy, if a pregnancy is <37 weeks (36 weeks 6 days or earlier) the Preterm field is incremented by 1 (Preterm = Premature) Note: Either a gestational age or the full term or preterm options must be documented for all outcomes with a gestational age >=20 weeks. Initial Prenatal Visits

With the Initial Prenatal Visit type, you can document the prenatal visit in the navigation tabs. The navigation tabs display based on which Obstetrics visit type. You can only change or select visit types from the Home or the Intake template. Automatic Calculation for Parity Values

You can correct and automatically calculate a patient’s current parity values. The automatic calculation of parity allows you to make corrections to a patient’s parity if they are incorrect. In the Pregnancy History grid, without adding the details of each associated pregnancy, you can quickly document a patient’s current parity values. You can also view all patients within the practice that have disparate parity detail information. The following is automatically calculated in the NextGen® Adaptive Content Engine system: Gravidity is the sum of full term, abortion, and ectopic pregnancies and are automatically calculated. A current pregnancy if any is calculated in this count. Parity is calculated in TPAL format (T for total number of full term births; P for total number of preterm births; A for total number of abortions; and L for total number of living children).
