Exceptions	Allows you to manage exceptions on the Exceptions/Measures template. This link appears in all panels except Fuctional and Cognitive Assessment Status.
Patient Education	Allows you to manage patient education resources on the Patient Education Browser window. The link appears only in the Health Promotion Plan panel.

Document Health Promotion Plans

You can document the Health Promotion Plans such as BMI plan, depression plan, and hypertension plan of a patient. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel and then select the Health Promotion Plan link.The Health Promotion Plan template opens. Expand the Health Promotion Plan panel. Add a diagnosis. Select Plan and select a plan from the list.Based on the selected plan, the different sets of fields appear. To add the plan details, do the following: For BMI Plan, add the diet, physical activity, and referrals details. For Depression Plan, dd the follow-up and referrals details. For Hypertension Plan, add the diet, physical activity, referrals, and lifestyle details. Note: To turn on the Referrals section and order a referral, you must add a diagnosis. To indicate that the referral is internal, select the Internal referral checkbox. If required, in the Details field, enter any additional information. If you want to add or update patient educat

Manage Health Promotion Plans

You can update and delete the Health Promotion Plans of a patient. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel, and then select the Health Promotion Plan link.The Health Promotion Plan template opens. Expand the Health Promotion Plan panel. Select a plan in the Additional Plans or Today's Health Promotion Plan Order's grid. Note: The Additional Plans grid displays the plans that are documented without a diagnosis. The Today's Health Promotion Plan Order's grid displays the plans that are documented with a diagnosis. To update the plan, make the required changes, and then select Update. Document Pain Assessment and Plan of Care

You can document the pain assessment details and follow-up plan of care. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel, and then select the Health Promotion Plan link.The Health Promotion Plan template opens. Expand the Pain Management panel. Note: The Pain level and Method fields display the details entered on the Vital Signs template. You cannot update the pain level and method in the Pain Management panel. Pain Management - Health Promotion Plan To manage exceptions and view measure details on the Exceptions/Measures template, select Exceptions. If details are not entered in the Pain level section on the Vital Signs template, select the following fields, and select the appropriate option in the list. Pain level Method Duration Quality To indicate to see the HPI details, select the See HPI checkbox. Enter the location of the pain of the patient. CQM Information Overview on the Health Promotion Plan Template

You can view the Clinical Quality Measures (CQM) information. The following table provides the CQM details of each panel on the Health Promotion Plan Template. Panel CQM Information Health Promotion Plan When a plan is selected in the Plan list, the following CQM information is displayed in detail, based on the plan selections. BMI Plan - CQM-BMI Plan-Adults or CQM-BMI Plan-Peds appear based on the age of the patient Depression Plan - CQM - Depression Plan Hypertension Plan - CQM - Hypertension Plan Pain Management CQM - Pain Assessment and Follow-Up. Document Functional and Cognitive Assessments

You can document a patient's functional and cognitive assessments in the Functional and Cognitive Assessment Status panel. The data entered in the Functional and Cognitive Assessment Status panel is shared with the Health Promotion Plan, Medicare Preventive HPI, Screening Summary, and Care Plan templates to measure compliance and for surveillance reporting. Open NextGen® Enterprise EHR. Open the Intake template. Expand the Vital Signs panel, and then select the Health Promotion Plan link. The Health Promotion Plan template opens. Expand the Functional and Cognitive Assessment Status panel. Functional and Cognitive Assessment Status For Functional Status and Cognitive Status, do the following: If there is no change in the functional status of the patient, select No Change. If there is a change in the functional status or cognitive status of the patient, select Change. Select the Status field, and select the appropriate option from the list. You can document a patient's health promotion plan on the Health Promotion Plan template. You can document the following details on the Health Promotion Plan template. BMI plan Depression plan Hypertension plan Pain management plan Fall risk Screening tools and follow-up care plans Functional and Cognitive Assessment Status Health Promotion Plan You can select the information icon to open the Information Display window, which displays all the details required for successful quality reporting. For adult patients, you can document the diet, physical activity, and referral details. For patients younger than 18 years, you must document the diet and physical activity details. The Exceptions link allows you to manage the exceptions on the Exceptions/Measures template. This link appears in all panels except Functional and Cognitive Assessment Status.
