Add Vital Signs on SOAP

While examining a patient, you can add the patient's vital signs in the Vital Signs panel on the SOAP template. Open NextGen® Enterprise EHR. Open the SOAP template. In Vital Signs, select Add. Review of Vital Signs on the SOAP Template

You can review and update a patient's vitals signs documented by the intake staff, in the Vital Signs panel on the SOAP template. You can add multiple vital signs for a patient during an encounter. The Vital Signs panel displays the most recent value for each vital sign. Options for Vital Signs The Add button opens the age specific vital signs template based on the patient age and the Pediatric Age configuration on the Practice Configuration template. Document Vital Signs

The provider reviews the vital signs added by the Intake staff on the Intake template and makes the applicable changes. Open NextGen® Enterprise EHR. On the SOAP template, expand the Vital Signs panel. Vital Signs Select Add. The age-specific vital signs template opens. Enter the necessary details on the template. Select Save. Note: Clear For Add is used to clear the existing data and add data. Select Close. To remove a record from the grid in the Vital Signs panel, select a record, and select Remove. Open the Vital Signs Template

You can document a patient's vital signs details on the Vital Signs template. Open NextGen® Enterprise EHR. To open the Templates module on a patient encounter, do one of the following: On the Categories or Encounter History tab, select Open Template. On Top Toolbar, select Templates. On the main menu, select File > New > Template. The Select Template window opens. Open the Intake template. In the Vital Signs panel, select Add.The Adult Vital Signs template opens. Edit a Reason for Visit on the SOAP Template

You can edit existing reasons for visits on the SOAP template. After the intake staff documents the patient's reasons for visit, you can edit the reasons for visit on the SOAP template to document additional details. Open NextGen® Enterprise EHR. On the SOAP template, expand the Reason for Visit panel. To edit a reason for visit, do the following: Select the HPI that you want to edit in the Reason for Visit grid. The corresponding HPI template opens. Edit the details in the HPI template. Select Save & Close. Review the Patient's Vital Signs

The Vital Signs panel shows the patient's vital signs as documented by the Intake staff. You can review and make the required changes to the documented vital signs by selecting a row in the grid and clicking the Edit button. To delete a vital signs record, select the row to be deleted and click the Remove button. The History link at the top of the grid shows the vital signs details in a tabular format.
