Filter for Barriers to Care
list.
The barriers available on the selected status appear in the grid.
Highlight a barrier in the grid and select
Edit
.
The
Barriers To Care
template opens.
Make relevant changes in the applicable fields.
Select
Save & Close
.
The modified details update in the grid.
To remove a barrier to care details from the grid, highlight a row in the grid and select
Remove
.

Document the Barriers To Care
In the Barriers to Care panel, select Add.The Barriers To Care template opens along with the calendar. Barriers To Care Select the onset date from the calendar. Select Barriers to care and select a reason from the Barrier Values list. Select Resolved Date and select the date if the barrier is resolved. Select Status and select the status of the barrier. Enter any Comments. Do one of the following: If you need to document additional barriers to care, select Save & Add New. Select Save & Close to return to the Barriers to Care panel. Barriers To Care Panel on the ASC - Pre admission Template
The Barriers to Care panel allows you to record and track patient circumstances that may negatively impact the patient’s ability to comply with the requirements of the care plan. Examples of barriers to care include such things as a lack of reliable transportation, difficulty communicating, other medical conditions that inhibit movement or understanding, family issues that make care difficult, and so on. The Barriers to Care panel is shared with other templates and specialties. This information carries forward to care management or family practice templates. Barriers to Care The clinical support staff can review the documented data and make the applicable changes. If the case ID of the encounters is the same, then the details documented in this panel on the ASC - Pre admission template carry forward to Barriers To Care panel on the ASC templates of the next subsequent encounters with the same case ID. Update a Barrier to Care
Open NextGen® Enterprise EHR. Select to highlight the barrier to care on the Barriers to Care panel that you want to update. Select the Edit button.The Barriers to Care template appears populated with data for the selected barrier to care. Update the fields on the template as required. Select the Save & Close button.You are returned to the Care Coordination Home Page with the selected barrier to care updated on the Barriers to Care grid. Record a Barrier to Care
Open NextGen® Enterprise EHR. Select the Add button for the Barriers to Care panel.The Barriers to Care template opens. Barriers To Care Select the Onset Date field and select the date the barrier to care began and select the OK button.The selected date is populated into the Onset Date field. Type a meaningful description of the barrier to care in the Barriers to care field. If the barrier to care has been resolved, select the Resolved Date field and select the resolution date from the resulting calendar. The Status field is pre-filled with a status of Active. To specify another status, select the field and select the new status from the resulting pick list. If you have resolved the issue that presented the barrier to care, you can select a status of Deleted. The barrier is marked as deleted but the information remains in the system for tracking purposes. Optionally, type additional comments in the Comments field. Remove a Barrier to Care
Open NextGen® Enterprise EHR. Select to highlight the barrier to care on the Barriers to Care panel that you want to delete. Select the Remove button.You are prompted to confirm the delete. Select the Yes button.You are returned to the Care Coordination Home Page with the selected barrier to care removed from the Barriers to Care grid.
