Document the Reason for Visit and HPI

You can document the reason for visit and HPI in the Intake template.

1. Open NextGen® Enterprise EHR.
2. Open the Intake template.
3. Open the Reason for Visit panel.
4. Select a complaint from the ten reasons for visit in the left pane.
   Note: In many practices, due to Medicare requirements, the intake staff does not enter information directly on an HPI template.
5. To add a reason for visit without opening the corresponding HPI template, select the Do not launch HPI option, and select the required reason for visit from the left pane.
   Note: If the required reason is not listed in the left pane, you can select the Additional / Manage link and select a reason on the Reason for Visit window.
   Note: You can configure the Do not launch HPI checkbox from the Practice Configuration template. For more information, go to NextGen Healthcare Success Community, and download the Practice Configuration Template User Guide for NextGen® Adaptive Content Engine.
6. To add reason for visit details in the corresponding HPI template, do the following:
   Note: Clear the Do not launch HPI checkbox.
   1. Select the required reason for visit from the left pane.
      The corresponding HPI template opens.
   2. Document the details in the template.
   3. Select Save & Close.
   Note: You can add maximum six HPIs to Reason for visit.
   The entered reasons appear in the right side list of the template.
