Document Passive Exposure

Recording passive smoke exposure is not a Quality Reporting requirement, nor does it record a patient’s smoking status in Quality Reporting. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, and select Add/Edit. In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit. The Social History – Tobacco template opens. Expand the Passive Exposure panel. In Have you ever had passive smoke exposure, select the applicable option. If you select No/Never, go to step 10. Otherwise, go to step 5. In Exposure in home environment, select Yes or No. In Other exposure locations, add other locations of passive exposure. In Tobacco type, select all tobacco types the patient is exposed to. In Length of exposure, enter how long the patient is exposed to passive smoke. Select a level of exposure. In Have you ever had passive vaping exposure, select

Document Patient's Current Tobacco Use Details

You can view and document the current tobacco use details for a patient. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, on the Social panel, select Tobacco/Vaping Use, and select Add/Edit. In the History Summary panel on Intake, select Tobacco/Vaping Use from Social, and select Add/Edit. The Social History – Tobacco template opens. Expand the Tobacco Use panel. Note: For the current encounter, NextGen® Enterprise EHR automatically selects the tobacco use option you selected for the previous encounter. For Have you ever used tobacco?, select No/never, Yes, or Unknown. If you select No/Never, then continue with Document Passive Exposure. Note: When you select No\Never for Have you ever used tobacco? then the system automatically selects No for the next two screening questions. Document Vaping Details of a Patient

The Tobacco/Vaping Use panel in the Social History - Tobacco template enables you to screen and document the vaping details of a patient. The documented vaping details are carried forward to future encounters. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, and select Add/Edit. In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit. The Social History – Tobacco template opens. Expand the Vaping Use panel. In the Screened for vaping field, select Yes. Select a Status for vaping. If the patient is a current user, select a number in the Age started field. If the patient is not a current user, select a number in the Age started and Age stopped fields. Document Tobacco Cessation Efforts

You can document a patient's tobacco cessation efforts in the Efforts to Quit Tobacco panel on the Social History – Tobacco template. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, and select Add/Edit. In the History Summary panel on the Intake template, select Social, select Tobacco/Vaping Use, and select Add/Edit. On the Social History – Tobacco template, expand the Efforts to Quit Tobacco panel. Select the Have you ever tried to quit using tobacco? option. If you select No/never, document details of the discussions on tobacco cessation by selecting the Tobacco cessation discussed link. Social History – Tobacco To add the details of the patient's efforts to quit tobacco, do the following: Select the tobacco type, and then enter the date the patient attempted to quit tobacco. Add Tobacco and Vaping Details

You can document the current tobacco use details and vaping details of a patient. Open NextGen® Enterprise EHR. To open the Social History – Tobacco template, do one of the following: On the Histories template, expand the Social panel, select Tobacco/Vaping Use, and click Add/Edit. In the History Summary panel on the Intake template, click Social, click Tobacco/Vaping Use, and then click Add/Edit. Social History – Tobacco Expand the Tobacco Use panel. Note: For a patient whose age is 12 and above, the smoking status must be recorded to meet the Quality Reporting requirements. Select the appropriate options for the questions, and document the details in the necessary sections according to the tobacco usage by the patient. Document vaping details in the Vaping Use panel and enter any comments. View Tobacco Use History

You can view the usage history of tobacco and smokeless tobacco products. Open NextGen® Enterprise EHR. To view the tobacco use history, do one of the following: On the Histories template, on the Social panel, open the Tobacco/Vaping Use template and expand Historical Use. In the History Summary panel on Intake, open Tobacco/Vaping Use from Social and expand Historical Use. If the patient has encounters entered prior to 7.9.1, select the Click here to see tobacco history prior to 7.9.1 link. The Tobacco Use History template opens. Select Save & Close.
