Document the Diaphragm/Cervical Cap Procedure Fitting Details

You can document the diaphragm or cervical cap fitting details in the Diaphragm/ Cervical Cap Fitting section on the Diaphragm/Cervical Cap - Procedure template.

1. Open the Diaphragm/Cervical Cap - Procedure template.
2. In the Diaphragm/Cervical Cap Fitting section, select a diagnosis in Indication for procedure.
3. Select a diagnosis status in Status.
4. Select the Physical Exam link, and then document the physical exam details.
5. Select Diaphragm or Cervical Cap.
6. Enter the type and size.
7. Select the following checkboxes as applicable.
   • Device fits appropriately
   • Patient practiced inserting and removing the contraceptive device in the office
   • Care of contraceptive device, use of spermicide jelly, and 6 hour waiting period before removal after intercourse were reviewed
   • All patient's questions answered
   • Education handout given to patient
